BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock BE and is hereby authorized and
directed to execute for and on behalf of the City of Lubbock a Professional
Services Agreement between the City of Lubbock and Texas Tech University,
attached herewith, which shall be spread upon the minutes of the Council
and as spread upon the minutes of this Council shall constitute and be a
part of this Resolution as fully copied herein in detail.
Passed by the City Council this 27th day of February , 1986.

Amendment to Contract No. MU19
Professional Services Agreement
The Agreement between City of Lubbock, Lubbock, Texas, and Texas Tech
University, Lubbock, Texas, dated February 28, 1985, is amended as
follows:
1. The term of this agreement shall be extended to February 1,
1987.
2. Paragraph I, Item F: If any fire fighter enrolled in this
fitness program does not bring from a licensed physician the
results of a physical exam performed within the last month
from the date of his evaluation under this program, then a
physical exam shall be required. This exam may be conducted
by the Department of Internal Medicine of Texas Tech Univer-
sity School of Medicine.
3. Paragraph V, Items 2 and 3: Family Medicine Department" will
be changed to "Department of Internal Medicine."
4. Paragraph VI, Add Item H: The Report of Physical Examination
for the Department of HPER Exercise Science Division of Texas
Tech University will consist of a focused clinical examination
directed toward assessing suitability of the candidate to
participate in a physical conditioning program. It is not
intended to be exhaustive and complete nor to screen for
occult malignancies, etc. This examination will not be per-
formed unless the subject brings with him the report of
medical history which he completes himself from the question-
naire that Dr. Bobo has provided all subjects. Following the
physical exam, clearance will be recommended or denied for
participation in an aerobic conditioning program. The exam-
ining physician's recommendation will be written on the
examination sheet and a copy retained in his hospital chart.
The original copy will be given to the fireman in a sealed
envelope to present to Dr. Bobo when he comes in for his
fitness evaluation at the HPER program. Because the physical
exam is obviously focused on medical clearance for partici-
pation in a physical conditioning program and because the
history is important in detecting some types of disorders
which would contraindicate vigorous exercise, the designated
physicians cannot complete the physical examination for
clearance without the medical history review.
In the event that the fireman appears for his physical exami-
nation without a completed medical history questionnaire,
Internal Medicine can either return him to the fire station to
be re -scheduled, or gather the requisite historical infor-
mation and fill out the history sheet at an additional cost of
twenty-five dollars to cover the additional time expended.
All other provisions of the Agreement will remain as written and all
parties do hereby ratify and confirm such terms, stipulations and
conditions therein set forth.
