HomeMy WebLinkAboutResolution - 4647 - Agreement- Lubbock Methodist Hospital System- Health Care Services For Employees - 10/20/1994Resolution No. 4647 October 20, 1994 Item #17 RESOLUTION BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK: THAT the Mayor of the City of Lubbock BE and is hereby authorized and directed to execute for and on behalf of the City of Lubbock a Hospital Service Agreement and all related documents by and between the City of Lubbock and Lubbock Methodist Hospital System for Health Care Services for City of Lubbock employees, which agreement is attached hereto, which shall be spread upon the minutes of the Council and as spread upon the minutes of this Council shall constitute and be a part of this Resolution as if fully copied herein in detail. Whereas, System owns and operates hospitals in Lubbock, Levelland and Plainview, Texas, known as Methodist Hospital, Methodist Children's Hospital, Methodist Hospital -Level land and Methodist Hospital -Plainview, which in the regular course of its business, furnish health care services to current, disabled, retired or other employees or former employees and any dependents of such persons eligible for coverage under City's health benefit plans and; Whereas, System desires to create inducements in the form of discounted pricing arrangement for services rendered to City employees or dependents and City desires to promote the System to its employees and dependents by making them aware of these inducements and encouraging them to utilize the System's facilities to the fullest practical extent and City will include financial incentives within its health benefit plan to use System; Now therefore, in consideration of the mutual covenants and agreements contained herein, City and System agree as follows: 1. Hospital Services - This Agreement shall cover all inpatient, outpatient and emergency room services provided by System hospitals named above. 2. Licensure/Accreditation - System represents and warrants that each of the above named hospitals are currently licensed under the laws of the State of Texas and that each is accredited by the Joint Commission on Accreditation of Healthcare Organizations. 3. Billing - System agrees to submit claims for covered persons to City payor. Such claim shall include a UB -92 billing form and all other items as instructed by City payor. 4. Payment - City payor shall make every effort to insure payment for services rendered is made within 30 days of receipt of a properly completed claim. 5. Term - The term of this Agreement will be for one- year from the date first written above. 6. Rate of Payment - System agrees to accept the rates in Exhibit A as payment in full for services rendered, including any co -payment by covered individuals. Billings at normal rates will be submitted to City payor and the Rate of Payment will be calculated and paid to System by payor. L Cit bbock, Ras B ' Name and Title: David R. Inpatient: Medical, per day $1,200 Surgical, per day 1,400 Step-Down/Telemetry, per day 1,500 ICU/SICU/CCU/NICU, per day 2,000 Normal Vaginal Delivery, package price, including well -baby 2,200 C -Section, package price, including well baby 3,250 Boarder Baby, per day 275 Rehabilitation, per day 750 Skilled Nursing, per day 375 Outpatient: Outpatient, percent discount from normal rates 25% Emergency, percent discount from normal rates 25%
