RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock BE and is hereby authorized and
directed to execute for and on behalf of the City of Lubbock, an Employer Agreement to
provide optional group legal services by and between the City of Lubbock and
LegalPlans USA, and related documents. Said agreement is attached hereto and
incorporated in this resolution as if fully set forth herein and shall be included in the
minutes of the City Council.
Passed by the City Council this 11th day of November 1999.

Agreement
LegalPlans USA will provide City of Lubbock with a Group Legal Services Policy, underwritten by Reliance National
Insurance Company. Subsequently, LegalPlans USA will issue a Certificate of Coverage to all participating employees.
This policy offers employee protection as it does not provide coverage for any action, proceeding, or dispute between the
employee and employer or their fellow employees.
The undersigned agrees to make the group legal plan available to all eligible employees. Furthermore, for each employee
who executes a payroll deduction request, the undersigned will withhold the amount authorized and forward to LegalPlans
USA all membership fees which are due and payable upon receipt of LegalPlans USA's monthly itemized billing statement.
The undersigned assumes no responsibility for forwarding membership fees from anyone other than current employees as
long as notification of terminated members is made to LegalPlans USA prior to the first day of the month following the
members' termination date.
The terms of the agreement are as follows:
Membership includes coverage for employee, spouse, and unmarried children under the age of 25 who reside in the
household or are full-time college students living outside the household as well as any physically or mentally disabled
dependents, regardless of age, living in the member's household.

This Agreement shall be automatically renewed on each anniversary date of this Agreement unless terminated by City of
Lubbock or LegalPlans USA as provided herein. This Agreement will commence on the first calendar day of the month
following the initial enrollment period. This agreement shall continue in force subject to earlier termination by LegalPlans
USA for the failure of the undersigned to pay the monthly membership fee, subject to a thirty-one day grace period or by
either party hereto upon sixty days written notice.
