RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock BE and is hereby authorized and directed to
execute for and on behalf of the City of Lubbock, an Amendment to Community Service Block
Grant Agreement, by and between the City of Lubbock and Contact Lubbock, and all related
documents. Said Amendment is attached hereto and incorporated in this resolution as if fully set
forth herein and shall be included in the minutes of the City Council.

AMENDMENT TO
COMMUNITY SERVICE BLOCK GRANT AGREEMENT
BETWEEN THE CITY OF LUBBOCK, TEXAS AND
CONTACT LUBBOCK INC.
This Amendment is entered into between the CITY OF LUBBOCK, TEXAS,
Texas Home -rule municipal corporation, (herein called the "City") and
CONTACT LUBBOCK , (herein called "Grantee").
WITNESSETH
WHEREAS, Lessor and Lessee entered into a written Agreement (the
"Agreement") on February 10, 2000 , for Community Service Block Grant
funding, which said Agreement is incorporated by reference;
WHEREAS, Lessor and Lessee wish to modify the Agreement to amend the
funding amount and extend the term.
NOW THEREFORE, THE PARTIES THEREFORE AGREE AS FOLLOWS:
The term of the Agreement shall be January 1, 1999 to December 31, 2000.
2. The funding of the Agreement shall not exceed $_50,000.00 .
Nor shall it exceed the remaining balance of 1999 finds $ 445.54
3. The scope of service of the Agreement shall be as follows:
Information & Referral, offer assistance to callers 24 hours a day, make referrals
to obtain emergency needs such as food, clothing, prescriptions, shelter,
utility and rental assistance. Projected number of calls to receive is 14,000.
4. The budget for the Agreement shall be as follows:

5. Except for the modification made in this Amendment to the Agreement, all
provisions of the Agreement shall continue in full force.
6. These modifications will become effective upon the date this Amendment is
signed.
