Said Change Order is attached hereto and incorporated in this resolution as if fully set forth herein and shall be included in the minutes of the City Council. After performance of a construction contract begins, Change Orders may be approved if necessary to 1) make changes in plans or specifications; or 2) decrease or increase the quantity of work to be performed or materials, equipment, or supplies to be furnished. This DOES NOT mean that a department can redefine the project. All work that alters the nature of the thing to be constructed or that is not an integral part of the project objective must be let out for public bid by the Purchasing Department. Description of change (alteration, deviation, addition, or deletion) caused by conditions encountered during construction not covered by the specifications and drawings of the project (attach additional ages if necessary): Please see attached 'Explanation. Memo and Itemized Cost spreadsheet Change Orders must either be negotiated in the best interest of the City of Lubbock or put out for public bid by the Purchasing Department. Where the Change Order is negotiated, the Change Order must be fully documented and itemized as to costs, including material quantities, material costs, taxes, insurance, employee benefits, other related costs, profit and overhead. Where certain unit prices are contained in the initial contract, no deviations are allowed in computing negotiated change order costs. SUBJECT: Milwaukee Avenue Change Order #4 DATE: February 1, 2001 PSC Job No: 01.2872.05 Council Members: This memo provides a detailed explanation of changes to the Milwaukee Avenue Extension Project Requested in Change Order #4. Item #1— Remove Item 126 — Loop Detectors The City Traffic Engineering Department has directed PSC to remove this item from the project in order to utilize advance camera detection in lieu of loop detecotrs. Eight loops at a cost of $3,000 each will be removed from the contract. This change will reduce the contract total by $24,000. Item #2 — Add Item 126A — Advance Detection Cameras This item will be added at the request of the City of Lubbock Traffic Engineering Department. This new item will read "Advance Detection Cameras on Signal Poles, including furnishing signal foundation and anchor bolts and all necessary conduit and cable. Shaft, arm, camera and cable (from controller to arm) furnished by owner." Four of these cameras will be installed on the project at a unit price of $3,000 each. This additional work will cause an increase to the contract of $12,000. Item #3 — Cost Reduction for Reduced Testing on Change Order #2 Change order #2 was accepted by the City Council on October 27, 2005. This change order added $411,200.00 to the construction cost of the contract based on the unit price of $11 per cubic yard for item #79. However, Change Order #1 included reduced quality control testing for this item which in effect reduced the unit price for the work. Change Order #2 cost was based on the full unit price for trench backfill remediation with no discount for reduced quality control testing. This additional work will cause an increase to the contract of $38,161.00. It is our understanding that Wal-Mart will reimburse the City of Lubbock for the cost of this work. Item #5 — Addition of Dual Left Turn lanes on US 62/82 The City of Lubbock requested that dual left turn lanes be constructed for the east and westbound traffic on US 62/82 at the 'intersection with Milwaukee Avenue. This work will consist of lengthening the left turn bays in both directions and widening the pavement to allow for two left turn movements. The addition of these lanes was approved by TxDOT Lubbock on January 20, 2006. This additional work will cause an increase to the contract of $67,852.50.
