HomeMy WebLinkAboutResolution - 2007-R0524 - Amendment To Employee Benefit Plan - 11/08/2007Resolution No. 2007-80524 November 8, 2007 Item No. 5.2 RESOLUTION BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK; THAT the Mayor of the City of Lubbock BE and is hereby authorized and directed to execute for and on behalf of the City of Lubbock, an Amendment to the Employee Benefit Plan, and related documents, and shall submit said Amendment and documents, if any, to the City's third party administrator for use in determining and administering employee benefits. Said Amendment is attached hereto and incorporated in this resolution as if fully set forth herein and shall be included in the minutes of the City Council. All capitalized terms shall have the same meaning as those terms are defined in the Plan Document. The following changes are hereby made to the Plan Document: 1. The Lifetime Maximum Benefit shall increase from $1,000,000 to $2,000,000. 2. The out -of -network, out-of-pocket maximums will decrease from $12,000 per Individual and $24,000 per Family, to $6,000 per Individual and $12,000 per Family. 3. Benefits for diabetic supplies (i.e., blood glucose test strips, lancets, blood glucose monitors, syringes, and needles) will be moved from the medical plan to the prescription drug plan. 4. Benefits for dental coverage will include athletic mouth guards, isolation of tooth with rubber dam, metal copings, mobilization of eruptedlmalpositioned tooth, precision attachments for partials and/or dentures and stress breakers, Benefits will be payable at 50%, after calendar year deductible, and will be subject to the $1,200 annual maximum.
