RESOLUTION
WHEREAS, the City Council of the City of Lubbock has determined that it is in the best
interest of the citizens of the City of Lubbock to enter into an agreement with Managed Care
Center for Addictive " Other Disorders, Inc., to participate in a cooperative effort to to help
individuals who are HIV positive and have a history of, or current problems with, substance
abuse by working together to to encourage and support these individuals to seek treatment, if
necessary, and to engage in activities that will focus their attention and energy on positive aspects
of life.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK THAT the Mayor
is hereby authorized and directed to execute the Memorandum of Agreement between the City of
Lubbock and Managed Care Center for Addictive i Other Disorders, Inc., and any associated
documents.

MEMORANDUM OF AGREEMENT
Managed Care Center for Addictive/Other Disorders, Inc. — HIV Early Intervention
Program (HEI) and City of Lubbock Health Department are both committed to assisting members of
our community in a cooperative effort by helping individuals who are HIV positive and have a history of
or current problems with substance abuse. We recognize that by working together we can encourage and
support these individuals to seek treatment, if necessary, and to engage in activities that will focus their
attention and energy on the positive aspects of life. To accomplish this goal we agree to the following:
Both parties will comply with all state and federal laws regarding confidentiality of information.
Either party may terminate this agreement without cause by giving written notice to the other party.
Managed Care Center, Inc. HIV Early Intervention Program will:
♦ Accept referrals for individuals who are HIV positive and report having a history of or current
substance abuse problems.
♦ Provide case management to address client needs including medical, substance use, and social
services.
♦ Provide access to substance abuse treatment as indicated.
♦ After obtaining proper consent in accordance with Federal Register 42CFR and HIPPA, HEI
Staff will share required information needed for referral and follow-up.
♦ Coordinate services with the City of Lubbock Health Department and other appropriate community
resources.
City of Lubbock Health Department will:
♦ Accept referrals from Managed Care Center for HEI clients needing services, if adequate staff are
available to provide services. If staff not available clients will be provided with an appropriate
referral.
♦ Educate individuals regarding the HEI program and offer a referral to those who are HIV positive
and have a history of substance abuse or current substance abuse problem.
♦ Provide HEI population with immunization and sexually transmitted disease testing according to the
current fee charged by the City of Lubbock Health Department.
♦ Present services to HEI population with the focused on education and prevention of communicable
diseases.
This agreement is effective September 1, 2013 and expires August 31, 2014.
