RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute for
and on behalf of the City of Lubbock, Change Order No.2 to that certain Contract No.12218 by
and between the City of Lubbock and Utility Contractors of America,Inc., for the South Lubbock
Sanitary Sewer Extension Phase 2-Bid Package A. and related documents. Said Change Order
No. 2 is attached hereto and incorporated in this resolution as if fully set forth herein and shall be
included in the minutes of the City Council.

"Change Order" means a written order to a Contractor, executed by the Owner, in accordance with the Contract authorizing an addition to, deletion
from, or adjustment or revision of the requirements of the Contract documents, or an adjustment to the compensation payable to the Contractor, or to
the time for performance of the Contract and completion of the project, or a combination thereof, which does not alter the nature of project and is an
integral part of the project objective. Adjustments to "Estimated Quantities" to a line time in a Unit Price Contract do not require a Change Order. All
work that alters the nature of the construction or that is not an integral part of the project objective must be let out for public bid.
Description of Change (alteration, deviation, addition, or deletion) caused by conditions encountered during construction not covered by the
snecifications and drawines of the Droiect (attached additional rages is MUM
Per attached backup material, Change Order #2 includes the following items: Item 99) Required item to connect sewer lateral to the
new manhole at the intersection of University Ave and FM 1585. Item 100) Required to re-work/densify subgrade of pavement on
University Avenue in front of Wal-Mart. A time extension of 105 days is included in this change order. A breakdown of the time
extension request is included in the backup material for this change order.
Where the Change Order is negotiated the Change Order most be fully documented and itemized as to costs, including material quantities, material
costs, taxes, insurance, employee benefits, other related costs, profit and overhead. Where certain unit prices are contained in the initial Contract, no
deviations are allowed in computing negotiated change order costs.

Dear Josh
As you are aware, the tie-in of the proposed University Avenue sewer line extension at the
intersection of University Avenue and FM 1585 and pavement revisions at University Avenue from
98'^ Street to 1001" Street are being added to this project. This change order and the following
description of the items are a result of this additional work for the above referenced project. Please
refer to the breakdown that is attached for the specific change of each item.
Item 99: This item will include excavating, chipping and removing the flowable fill and concrete
encasement, cutting the fiberglass tee base, and performing the fiberglass patch for the new lateral
sewer line. This change will result in an increase of $14,500.00.
Item 100: This item includes re -working and re-densifying the subgrade for the pavement along
University Avenue in front of the new Wal-Mart. The contractor was stopped so that additional
pavement width could be designed during which time the subgrade went out of compliance. This
change will result in an increase of $28,000.00.
The change in contract price associated with these changes is detailed below:
• Original contract price: $11,921,273.80
• Net change of this change order: $42,500.00
• Amount of previous change orders: $561,338.00
• Contract price with all approved change orders: $12,525,111.80

The work outlined above was part of the critical path for the project. Other items included in the time
extension are: re -alignment of a portion of the roadway, additional concrete pavement work, and
weather days. This work is expected to take an additional 105 days that are recommended to be
added to the contract. A breakdown of the time extension request from the contractor is attached.
• Original contract time: 300 days
• Original NTP date: June 15, 2015
• Contract time change with this change order: 105 days
• Contract time change of previous change orders: 108 days
• Contract time with all approved change orders: 513 days
• Revised Substantial Completion Date: November 9, 2016
