RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute for
and on behalf of the City of Lubbock Amendment No. 5 to Professional Services Agreement No.
12234 for Architectural/Engineering Services at LPSIA, by and between the City of Lubbock and
RS&H, Inc., of Houston, Texas, and related documents. Said Amendment is attached hereto and
incorporated in this resolution as if fully set forth herein and shall be included in the minutes of
the City Council.

EAST GA RAMP REHABILITATION
PROFESSIONAL SERVICES WORK PLAN
The following Work Plan is provided to the City of Lubbock (henceforth the "CITY") Preston Smith
International Airport (henceforth the "AIRPORT" or "OWNER") by RS&H, Inc. (henceforth the
"ENGINEER") for professional design and construction administration services for the completion of the
East GA Ramp Rehabilitation (henceforth the "PROJECT').
This Work Plan describes the scope of services to be provided by the ENGINEER for the PROJECT. This
Work Plan includes a PROJECT Schedule and a PROJECT Proposal. The PROJECT Schedule defines
the durations of work that are the basis of the Work Plan and PROJECT Proposal. The PROJECT
Proposal provides the basis of the estimate for the required compensation and defines the task efforts
and deliverables required for the PROJECT.
PROJECT DESCRIPTION
Background — The East GA Ramp, located adjacent to and east of Runway 17L -35R, is in various
stages of pavement deterioration. The ramp serves as access to Chaparral Jet Center and various
Airport owned box and T -hangars. The ramp also serves as overflow parking for General Aviation aircraft
during peak demands. Portions of the ramp are constructed of an asphalt surface course with significant
indications of pavement failure. Other portions, constructed of concrete pavement, also show visual
indications of pavement deterioration with exception of the 50' wide taxilane and a 300' by 100' apron
directly west of Chaparral Jet Center.

A Preliminary Engineering Report (PER) was completed under Contract No. 12234, Amendment No. 3
(Work Area A) and Amendment No. 4 (Work Area B), shown in Figure 1, with a goal to provide the most
and best use of the ramp during normal and peak demand periods. Various ramp configurations and
reconstruction methods were evaluated. The studies catalogued within the PER included the
ENGINEER's assessments, corresponding construction budget estimates, and recommended design

solutions. Upon completion of the PER, a preferred alternative was chosen by the OWNER to be the
basis of design of this PROJECT.
Additionally, a portion of the airfield perimeter road adjacent to the southwest corner of the Terminal
Apron, shown in Figure 2, has been damaged such that it is no longer a usable roadway surface. It is
believed that potentially trapped water has degraded the pavement material qualities and the strength of
the pavement layers. The combination of presumed high moisture levels in the pavement section,
assumed pavement material degradation and heavy traffic loading (i.e. fuel trucks) has caused
irreparable pavement distresses.

Scope of Work — The PROJECT can generally be described as:
• The rehabilitation of the AIRPORT's East GA Ramp (Work Area A only) through mill and overlay
of the existing hot -mix asphalt (HMA) surface, as documented in the previously completed PER.
The PROJECT scope of work is based on providing the services described herein.
• The complete reconstruction of the damaged portion of the airfield perimeter road adjacent to the
southwest corner of the Terminal Apron. A primary objective for this pavement repair is to
address the underlying water issues to prevent the new paving materials from being saturated or
being exposed to high moisture levels for extended periods of time. The scope of this work will
not include the study and location of the source of the presumed trapped water, but shall instead
only include the removal of trapped waters via subsurface underdrains.
It is assumed that the PROJECT will be designed as one (1) set of Bidding Documents and will be
constructed as one (1) construction project. The PROJECT will be designed to meet the current
requirements of the Federal Aviation Administration (FAA), the OWNER, and the City. If standards
conflict, the most stringent standard shall govern. Any deviations from these assumptions once notice has
been given to the ENGINEER to begin the work of this contract may require a change to the PROJECT
scope and additional fees.
The ENGINEER will generically provide the following services (detailed task descriptions are presented
herein):

o Consult with the OWNER to clarify and define the scope of the PROJECT, design
requirements for the PROJECT, and review available data.
o Identify and analyze requirements of governmental authorities having jurisdiction to
approve the design of the PROJECT and participate in consultations with such
authorities.
o Prepare design documents for the following milestones: 1) 90% Design Review and 2)
Sealed Bidding Documents. Bidding Documents shall consist of technical drawings,
technical specifications, Contract Documents, and the ENGINEER'S Opinion of Probable
Construction Cost (EOPC).
o Prepare a detailed work schedule and phasing plan and develop a Construction Safety
Phasing Plan (CSPP).
o Design and plans shall be prepared using Auto CAD Civil 3D 2013.
o Additional requirements of the ENGINEER are as set forth in Exhibit A of the Professional
Services Agreement, Resolution No. 2015-R0062.
C Safety Risk Management (SRM) and Safety Assessment
o Coordinate and moderate a Safety Risk Management (SRM) panel and prepare all
follow-up Safety Assessment documentation.
O Bidding Phase
o Assist the OWNER in advertising for and obtaining bids or proposals for the PROJECT.
o Conduct a pre-bid conference.
o Issue addenda as appropriate to clarify, correct, or change the Bidding Documents.
o Attend the bid opening, prepare bid tabulation sheets, and assist the OWNER in
evaluating bids or proposals and in assembling and awarding contracts for the work of
the PROJECT.
o Additional requirements of the ENGINEER are as set forth in Exhibit A of the Professional
Services Agreement, Resolution No. 2015-R0062.
C Permitting
o Prepare the following permits and/or authorizations, though all associated fees have
been excluded from this contract, unless otherwise noted:
y Site Plan Review;
Building Permit Application; and
➢ Notice of Proposed Construction or Alternation, in accordance with the FAA's
Obstruction Evaluation/Airport Airspace Analysis (OE/AAA) requirements.
C Construction Administration Phase
o Consult with the OWNER and act as the OWNER's representative throughout
construction of the PROJECT.
o Prior to commencement of construction operations, the ENGINEER shall conduct a pre -
construction conference attended by the OWNER, Contractor, and other governmental
authorities, as needed.
o Make visits to the site at intervals appropriate to the various stages of construction as the
ENGINEER deems necessary in order to observe as an experienced and qualified design
professional the progress and quality of the various aspects of the Contractor's work.
o Provide the services of a full-time Resident Project Representative (RPR) at the
PROJECT construction site to assist the ENGINEER and to provide more extensive
observation of the Contractor's work. Duties, responsibilities, and limitations of the RPR

are as set forth in Exhibit C of the Professional Services Agreement, Resolution No.
2015-R0062.
o Provide the services of an independent testing laboratory to perform all inspections, tests,
and approvals of samples, materials, and equipment required by the Contract
Documents, or to evaluate the performance of materials, equipment, and facilities of the
OWNER, prior to their incorporation into the work of the PROJECT with appropriate
professional interpretation thereof.
o Provide construction survey verification services in order to determine if construction
work has been generally completed in general conformance with the Contract
Documents.
o Attend a Final Inspection with the Contractor and OWNER representatives to determine if
construction work has been completed in general conformance with the Contract
Documents and prepare a list of visually non -conforming items and un -completed items
of work.
o The ENGINEER shall not be responsible for the acts or omissions of the Contractor, or of
any subcontractor, any supplier, or of any other person or organization performing or
furnishing any of the work unless the work is done in conformance with the Contract
Documents of the ENGINEER. The ENGINEER shall not be responsible for the
Contractor's failure to perform or furnish the work in accordance with the Contract
Documents.
o Additional requirements of the ENGINEER are as set forth in Exhibit A of the Professional
Services Agreement, Resolution No. 2015-R0062.
a Post -Construction Phase
o Prepare PROJECT Record Drawings from as -constructed drawings provided by the
Contractor and RPR.
The A/E services are divided into two (2) categories: Basic Services and Special Services. Basic
Services are those services recognized as necessary A/E efforts performed in direct support of
the production of Bidding Documents. Special Services are efforts which further the progress of
the PROJECT but are not a direct participation in the document production for the basic facility.
I1. BASIC SERVICES
Task A1.01 Study and Report Phase Services
This phase is not applicable to this PROJECT as it has been previously completed under Contract No.
12234, Amendment No. 3 (Work Area A) and Amendment No. 4 (Work Area B).
Task A1.02 Preliminary Design Phase Services
This phase is not applicable to this PROJECT due to the relatively straightforwardness of the PROJECT.
Task A1.03 Final Design Phase Services
Performance of this task will be based on the preferred alternative chosen by the OWNER as noted in the
PER and shall define the design and establish the scope, relationships, forms, size and appearance of
the PROJECT through plans, sections, elevations, details and specifications. Specific task breakdowns
are as follows:
Task A1.03.1 Participate in Design Kick-off Meeting
The ENGINEER shall coordinate and attend one (1) design kick-off meeting with the OWNER to
clarify and define the scope of the PROJECT, design requirements, and methods for the PROJECT.
The ENGINEER will also review PROJECT requirements with the OWNER and other stakeholders as

identified and recommended by the OWNER. This meeting is , anticipated to be held via
teleconference and will not require travel by the ENGINEER.
Task A1.03.2 Perform Data Collection and Research
The ENGINEER shall complete data gathering, review FAA design guidelines and review existing
records, maps, plats, aerial photographs and utility information in the OWNER's files and collect all
available information related to the PROJECT.
The ENGINEER shall also identify and analyze requirements of governmental authorities having
jurisdiction to approve the design of the PROJECT and participate in consultations with such
authorities, as required.
Task A1.03.3 Perform Project Management and Administration
The ENGINEER shall manage the PROJECT in a professional manner, assign and manage qualified
individuals or subconsultants to the PROJECT, and complete the efforts within the proposed time
frame.
The ENGINEER shall complete meeting preparation activities, prepare and distribute meeting
minutes, and assign appropriate follow-up activities.
This task shall include weekly teleconference meetings with the OWNER to discuss PROJECT
progress.
Task A1.03.4 Prepare 90% Design Documents
The ENGINEER shall prepare 90% plans, Project Manual (Contract Documents and technical
specifications), and EOPC. The design shall evaluate and identify specific elements of the PROJECT
for a technically and economically sound project.
Task A1.03.5 Prepare Construction Safety Phasing Plan (CSPP)
The ENGINEER shall prepare a CSPP in conformance with FAA AC 150/5370-2, Operational Safety on
Airports during Construction, latest edition.
Task A1.03.6 Perform In -House Quality Control Review— 90% Design Documents
The ENGINEER shall conduct an in-house quality control review of the 90% Design Documents prior to
submittal to the OWNER.
Task A1.03.7 Submit 90% Design Documents to the OWNER
The ENGINEER shall submit and distribute copies of the 90% Design Documents to the OWNER for
review, comment, and approval to proceed with preparation of Bidding Documents. Distribution
quantities of the 90% Design Documents shall be as noted in the DELIVERABLES section of this
Proposal.
Task A1.03.8 Participate in 90% Design Documents Review Meeting with the OWNER
The ENGINEER shall coordinate and attend one (1) 90% Design Documents Review Meeting with
the OWNER to receive comments and direction regarding the 90% Design Documents. This meeting is
anticipated to be held via teleconference and will not require travel by the ENGINEER
Task A1.03.9 Incorporate 90% Design Documents Review Comments
The ENGINEER shall review all comments received from the OWNER, SRA panel participants (as
described in SPECIAL SERVICES section of this Proposal), and the CITY Site Plan Review committee
for the 90% Design Documents submittal review and incorporate applicable comments into the design
documents and CSPP. The ENGINEER shall provide a written response for each comment on how it
will be incorporated into the design documents or why it was not applicable.
Task A1.03.10 Prepare Sealed Bidding Documents
The ENGINEER shall prepare sealed Bidding Documents (plans and Project Manual) and a final
EOPC.

Task A1.03.11 Perform In -House Quality Control Review — Sealed Bidding Documents
The ENGINEER shall conduct an in-house quality control review of the sealed Bidding Documents
prior to submittal to the OWNER.
Task A1.03.12 Submit Sealed Bidding Documents to the OWNER
The ENGINEER shall submit and distribute copies of the sealed Bidding Documents to the OWNER.
Distribution quantities of the sealed Bidding Documents shall be as noted in the DELIVERABLES
section of this Proposal. Comments requesting changes to the sealed Bidding Documents after this
submittal will be performed under a change order for additional time and cost, unless due to an error or
omission by the ENGINEER.
Task A1.04 Bidding Phase Services
The services commencing after completion and submission of the Bidding Documents are anticipated to
include assistance during bidding, issuance of addenda as necessary, pre-bid conference attendance,
and bid evaluation. The ENGINEER has included one (1) Bidding sequence. Specific task breakdowns
are as follows:
Task A1.04.1 Provide Bidding Assistance
The ENGINEER shall assist the OWNER in advertising for and obtaining bids or proposals for the
PROJECT.
Task A1.04.2 Attend Pre -Bid Conference
The ENGINEER shall attend and conduct one (1) pre-bid conference.
Task A1.04.3 Issue Addenda
The ENGINEER shall issue addenda as appropriate to clarify, correct, or change the Bidding
Documents.
Task A1.04.4 Evaluate Bids
The ENGINEER shall not attend any bid opening. Contractor bids shall be provided to the ENGINEER
by the OWNER for evaluation. After the bid opening, the ENGINEER shall develop a tabulation of all
bids received and provide evaluation of such, checking for correctness, qualifications of apparent low
bidder, DBE participation goals, etc., and make recommendations of award based solely on apparent
low bidder. The ENGINEER cannot and does not guarantee that bids will not vary from the opinion of
probable cost.
Task A1.05 Construction Phase Services
The services anticipated to be rendered after commencing the Construction Phase are as follows:
Task A1.05.1 Provide General Administration of the Construction Contract
The ENGINEER shall consult with the OWNER and act as the OWNER's representative throughout
construction of the PROJECT.
This task shall include providing PROJECT oversight; general correspondence by the ENGINEER
with the OWNER and / or Contractor; coordination of meetings with the OWNER, Contractor, and / or
other governing authorities; completion of meeting preparation activities, preparation and distribution
meeting minutes, and assignment of appropriate follow-up activities; PROJECT reporting;
coordinating, receiving, and reviewing Contractor completion documents; and PROJECT record
keeping.
This task shall include weekly teleconference meetings with the OWNER and Contractor to discuss
PROJECT progress.
The ENGINEER shall manage the PROJECT in a professional manner, assign and manage qualified
individuals or subconsultants to the PROJECT, and complete the efforts within the appropriate time
frame.

Task A1.05.2 Attend Pre -Construction Conference
The ENGINEER shall attend and conduct one (1) pre -construction conference.
Task A1.05.3 Attend Site Visits
The ENGINEER shall, in connection with observations of the work of the Contractor while it is in
progress, make visits to the construction site at intervals appropriate to the various stages of
construction as the ENGINEER deems necessary in order to observe and familiarize themselves
generally with the progress and quality of the work and to determine if the work is generally
proceeding in accordance with the Contract Documents and the Contractor's construction schedule.
The ENGINEER shall make periodic written reports to the OWNER to advise of any deviation from
the Contract Documents or the Contractor's construction schedule observed by or brought to the
attention of the ENGINEER. The ENGINEER shall additionally make written reports of
recommendations to the OWNER regarding any work completed by the Contractor that the
ENGINEER has determined to be defective, that will not produce a completed PROJECT generally
conforming to the Contract Documents, and / or that will prejudice the integrity of the design concept
of the completed PROJECT as a functioning whole. This Proposal includes two (2) site visits to be
performed by the ENGINEER, not including Final Inspection. Should the requirement for additional
site visits be necessary, a change to the PROJECT scope and additional fees may be required.
The construction duration is anticipated to take approximately 60 days. The schedule will be refined
during design and modification to the fee may be required.
The ENGINEER shall not have control or charge of and shall not be responsible for construction
means, methods, techniques, sequences or procedures or for the safety precautions and programs in
connection with the project construction, for the acts or omissions of the Contractor, subcontractors,
any of their agents or subcontractor's employees, or any other person performing any of the work or
for the failure of such persons to carry out the work in accordance with the Contract Documents.
Task A1.05.4 Review Contractor Submittals, Shop Drawings, and Samples
The ENGINEER shall review and approve (or take other appropriate action) upon the submittals,
shop drawings, samples, and other submissions furnished by the Contractor and submitted to the
ENGINEER. The ENGINEER shall maintain a log of all Contractor submittals which shall include the
submittal date, the action taken, and the date returned.
The ENGINEER shall evaluate Contractor proposals and substitutions and recommend to the
OWNER to either approve or disapprove the Contractor's proposal or substitution.
Task A1.05.5 Provide Clarifications and Interpretations
The ENGINEER shall prepare and provide design clarifications, interpretations, and
recommendations to assist the OWNER in resolving field problems relating to construction. Such
clarifications and interpretations shall be consistent with the intent of and reasonably inferable from
the Contract Documents. The ENGINEER may, as appropriate, issue Field Orders authorizing minor
variations from the requirements of the Contract Documents.
Task A1.05.6 Prepare Change Orders and Work Change Directives
The ENGINEER shall prepare routine change orders as required and act as interpreter of the terms
and conditions of the Contract Documents and judge of the performance thereunder by the parties
thereto and make decisions on claims of the OWNER and Contractor relating to the execution and
progress of the work and other matters and questions related thereto.
Task A1.05.7 Review Independent Testing Laboratory Reports
The ENGINEER shall review all reports of inspections, tests, and approvals of samples, materials,
and equipment by an independent testing and provide appropriate professional interpretation thereof.
Task A1.05.8 Review Applications for Payment
The ENGINEER shall review Contractor applications for payment and supporting data, review the
amount owed to the Contractor and recommend approval in writing all payments to the Contractor in
accordance with the Contract Documents.

Task A1.05.9 Attend Final Inspection
The ENGINEER shall attend one (1) Final Inspection with the Contractor and OWNER
representatives to determine if construction work has been completed in general conformance with
the Contract Documents and prepare a punchlist of visually non -conforming items and un -completed
items of work. Once the ENGINEER has determined the work of the PROJECT to be substantially
complete, the ENGINEER shall deliver a Certificate of Substantial Completion to the OWNER and
Contractor. Once the Final Inspection punchlist items have been completed by the Contractor and the
ENGINEER has determined the work of the PROJECT to be completed in general conformance with
the Contract Documents, the ENGINEER shall provide to the OWNER a recommendation of Final
Payment to the Contractor.
III. SPECIAL SERVICES
Task A2.01 Design Surveys
Task A2.01.1 Obtain Design Survey
The ENGINEER shall obtain a topographical survey as required to perform the design of the
reconstruction of the damaged portion of the airfield perimeter road adjacent to the southwest corner
of the Terminal Apron.
Design surveys shall be performed by Hugo Reed and Associates, Inc. (HRA). HRA's proposal and
scope of work for these services has been included in Attachment B.
Design surveys for the East GA Ramp have already been performed under Contract No. 12234,
Amendment No. 3 (Work Area A) and Amendment No. 4 (Work Area B).
Task A2.02 Safety Risk Management System (SRM) and Safety Assessment
Task A2.02.1 Review and Analysis of Relevant Safety Information
The ENGINEER shall review and analyze the following: operational phasing plans; specific PROJECT
construction safety and phasing plans; potential impacts to safety and operational safety associated
with CFR Part 139 compliance with other aviation related areas based on the specific PROJECT safety
and phasing plans; positive control escort procedures; foreign object debris (FOD) plan; Airport
Operations Area (AOA) ingress/egress routes; aircraft movement areas (AMA); coordination and
security; markings, signage, winter weather operations related to the construction phases; emergency
access routes, construction staging areas; vehicles crossing active pavements procedures; NOTAMs;
jet blast -prop -wash safety; night-time construction lighting; dust control plan; hauling of debris;
equipment parking; interference with NAVAIDS; notification plans to airlines, FAA, and other tenants;
aircraft taxi routes; and potential impact to low -visibility routes — Surface Movement Guidance Control
System (SMGCS).
The analysis will not address the study of visibility — line of sight issues involving construction
equipment, the analysis of interference with communications and surveillance equipment, or
environmental studies. However, the safety risk management analysis will use the results of those
studies, prepared by others, and OWNER provided Subject Matter Experts (SMEs) in these areas as
may be required to conduct a Functional Hazard Analysis (FHA).
Taste A2.02.2 Conduct a Safety Risk Management (SRM) Panel
The ENGINEER shall coordinate and moderate a SRM panel and prepare all follow-up
correspondence.
The proposed approach for safety risk assessment and management will follow the guidelines
described in FAA Order 5200.11 FAA Airports (ARP) Safety Management System, and other applicable
FAA Guidance. The information will be processed and the stakeholders will suggest risk control
measures. The ENGINEER shall draft a report outlining the hazards identified, the existing controls,
initial risk ranking, additional requirements, final risk ranking, and mitigation monitoring (if necessary).

Task A2.02.3 Prepare and Submit a Safety Assessment
The ENGINEER shall prepare and submit all Safety Assessment documentation, including findings of
the SRM panel and reviews of all relevant safety information. The Safety Assessment shall follow the
guidelines described in FAA Order 5200.11 FAA Airports (ARP) Safety Management System.
Task A2.03 CITY Permit Approval
Task A2.03.1 Prepare and Submit CITY Site Plan Review Application
The ENGINEER shall prepare the CITY Application and Checklist for Site Plan Review. The
ENGINEER shall then submit the Application and copies of the 90% Design Documents to the CITY
Site Plan Review committee in order to initiate the CITY Building Permit review process. Distribution
quantities of the 90% Design Documents shall be as noted in the DELIVERABLES section of this
Proposal.
Task A2.03.2 Attend CITY Site Plan Review Meeting
The ENGINEER shall coordinate and attend one (1) CITY Site Plan Review committee meeting to
receive comments and direction regarding the 90% Design Documents.
Task A2.03.3 Prepare and Submit CITY Site Commercial Building Permit Application
The ENGINEER shall prepare the CITY Commercial Building Application. The ENGINEER shall then
submit the Application and copies of the sealed Bidding Documents to the CITY Building Inspection
Department for review. Distribution quantities of the sealed Bidding Documents shall be as noted in the
DELIVERABLES section of this Proposal.
Task A2.04 Obstruction Evaluation / Airport Airspace Analysis (OE/AAA) Approval
Task A2.04.1 Prepare and Submit FAA Form 7460-1
The ENGINEER shall file a Notice of Proposed Construction or Alteration (FAA Form 7460-1) for the
PROJECT electronically to the FAA for OE/AAA review and approval.
Task A2.05 Resident Project Representative (RPR)
Task A2.05.1 Provide RPR Services
The ENGINEER shall provide the services of a part-time RPR at the PROJECT construction site to
assist the ENGINEER and to provide more extensive observation of the Contractor's work. Duties,
responsibilities, and limitations of the RPR are as set forth in Exhibit C of the Professional Services
Agreement, Resolution No. 2015-R0062.
RPR services shall be performed by Terracon. Terracon's proposal and scope of work for these
services has been included in Attachment D.
Task A2.06 Construction Verification Surveys
Task A2.06.1 Obtain Construction Verification Surveys
The ENGINEER shall provide construction verification survey services, as necessary, in order to
determine if construction work has been generally completed in general conformance with the
Contract Documents.
Construction verification surreys shall be performed by Hugo Reed and Associates, Inc. (HRA).
HRA's proposal and scope of work for these services has been included in Attachment C.
Task A2.07 Independent Testing Laboratory
Task A2.07.1 Provide Independent Testing Laboratory Services
The ENGINEER shall provide the services of an independent testing laboratory to perform all
inspections, tests, and approvals of samples, materials, and equipment required by the Contract
Documents, or to evaluate the performance of materials, equipment, and facilities of the OWNER,
prior to their incorporation into the work of the PROJECT with appropriate professional interpretation
thereof.

Independent testing laboratory services shall be performed by Terracon. Terracon's proposal and
scope of work for these services has been included in Attachment E.
Task A2.08 Post -Construction Phase Services
Task A2.08.1 Prepare PROJECT Record Drawings
The ENGINEER shall prepare PROJECT Record Drawings, which shall become the property of the
OWNER, corrected to show significant changes made in the work during the construction of the
project. Such corrections shall be based upon "as -built" prints, drawings, field sketches and other
data furnished to the ENGINEER by the RPR and the Contractor, and upon change orders issued
during construction.

1. Project Information
Proposal PAR161047 / RPR Services Client indicated that resident project representative (RPR) services may be needed for the project.
2. Scope of Services The scope of Services to be provided under this Task Order are described in the Scope of Services section of the Consultant's
Task Order Proposal, unless Services are otherwise described below or in Exhibit B to this Task Order.
Proposal PAR161047 / RPR Services: The scope for (rosponsibililies of) the RPR hasn't been defined. Based on discussion with Client, (Ile role will be
limited to making observations on behalf of and reporting observations to the Engineer (Client). The RPR will not have decision making authority It is
understood the RPR role will be clearly defined prior to contact execution.
3. Compensation Client shall pay compensation for the Services performed at the fees staled in the Task Order Proposal unless fees are otherwise
slated below or in Exhibit C to this Task Order.
Proposal PAR161047 / RPR Services: The ability to provide an RPR for the project will depend on 1) what the actual responsibilities will be
defined to be and 2) the availability of a person who can fulfill those responsibilities. Assumptions: Project will start in July 2017, 60 to 90 calendar
day project duration. an average of 20 hours per week for 90 calendar days, an experienced graduate engineer will serve in the RPR role.
250 hours x $100.00/hour = $25,000 00 (estimate : hourly rate is subject to change upward or downward if it's agreed that an experienced
graduate engineer isn't appropriate for the RPR role).
All terns and conditions of the Master Services Agreement shall conlinoe in full force and effect This Task Order is accepted and Consultant is

Terracon Consultants, Inc. appreciates the opportunity to submit this revised proposal to provide
construction materials testing services for the referenced project. This proposal outlines our
understanding of the scope of services that may be provided by Terracon and includes the unit fees
and a cost estimate for services we anticipate will be requested for this project.
A. PROJECT INFORMATION
At the time this proposal was prepared, the following information was known:
The project has not yet been designed.
The project will consist of -23,450 SY of two inch milling and overlay.
FAA P-401 HMA will be the governing specification (although the spec hasn't yet been customized
for this project.
A test section will be required (assumed to be a separate / 6th lot).
Five days of asphalt production are assumed (five lots). Two days are added for a contingency.
Construction is projected to occur in July 2011.

E. SCOPE OF SERVICES
Terracon proposes to provide the following scope of services based on our review of the information
provided by Client to dale and on recent e-mail and phone discussions with Client.
A. Asphalt Laboratory Testing at the testing facility provided by others at the
asphalt plant for the use of the Engineer (spec 401-4.2 b).
On a call -out basis, perform asphalt mixture tests as required by the
specifications. The tests will be performed by a TxDoT certified technician, Level
1A (Plant Production Specialist).
B. Asphalt Laboratory Testing- at Terracon's Lubbock, TX laboratory (spec 401-5.1).
On an as -requested basis, measure thickness of and perform bulk specific
gravity tests for asphalt cores secured by the contractor. We assume that
thickness and density will be determined for twenty-four mat cores and twenty-
four joint cores.
C. Observe Coring By Contractor & Test The Cores In The Lab
On a call -out basis, observe coring performed by the contractor (at locations
selected by others), return the cores to the lab, measure core thicknesses,
prepare (saw) the cores and test the cores for bulk density.
In order to provide effective service, we ask that current plans and specifications be provided to
Terracon at least one week days prior to the first site/plant visit. We ask that contract document
revisions be provided as applicable.
We anticipate providing materials testing services for this project on a call -out basis. We understand
the client or his/her designated representative will schedule our services. We recommend that the
person(s) who schedule(s) our services be made aware of the types and quantities of services
included in our scope. We ask that service requests be made by 4PM on the business day (Monday -
Friday) prior to the service date. When "same day" service requests are received, we'll do the best
we can to accommodate. We ask that service requests not be made by text message or through
the field technician. If scheduling by e-mail, please don't assume the service has been scheduled
until you receive a confirmation reply from us.
G. PROJECT STAFFING AND ADMINISTRATION
Terracon's approach to providing materials testing services is to assign qualified engineering
technicians, directed by Professional Engineers licensed by the Slate of Texas, to perform the

requested tests for your project. Most of our technicians are certified by the American Concrete
Institute (ACI) and many are also certified by other entities. Our technicians participate in internal
training programs and receive continuous in-house instruction. Terracon's Project Manager's
responsibilities will include:
Review field and laboratory testing activities,
Ascertain whether or not test results comply with project requirements,
Distribute deviation log as directed by Client,
Communicate with our field technicians, the Contractor, the Engineer and the Owner's
representative,
Review and distribute laboratory test reports,
Keep Client apprised of the status of the testing budget and
Prepare / submit invoices as directed by Client.
D. SITE SAFETY
At Terracon, we have a personal and uncompromising commitment to everyone going home safely
every day. Our Incident and Injury -Free (IIF) culture is about care and concern for people. As part of
our IIF program, we will prepare a Pre -Task Plan to address potential site safety hazards. The intent
of the Pre -Task Plan is to enable our personnel to assess unsafe conditions and identify potential
hazards. If we observe unsafe behaviors, we will discuss them with the general contractor's
superintendent. Terracon will be responsible for supervision of and safety measures for its own
employees but we will not be responsible for supervision of personnel employed by the Client, general
contractor, subcontractors or other parties present at the site.
I _. COMPENSAT'ION'
Our cost estimate to provide the scope of services described herein is $13,988.00. An itemized cost
estimate is attached (see Exhibit B). Please note we will only invoice for actual services rectuested
/ provided. If the estimated quantities are exceeded due to service requests that exceed those
anticipated, additional fees will apply. Services not included in this proposal, which may be
requested by Client or Client's authorized representative, will be invoiced using the unit rates and
conditions on the attached fee schedule (see Exhibit A).
Hourly rates will be charged portal to portal from our Lubbock, Texas office There is a minimum
charge of four hours per trip for field services. Overtime rates (1.5 times regular rales) will apply to
time in excess of eight hours per day (Monday — Friday), hours before 7AM or after 5PM (M -- F)
and all hours on Saturdays. The overtime factor is 2.0 for work on Sundays and holidays.
