Description of change (alteration, deviation, addition, or deletion) caused by conditions encountered during construction not covered by the specifications and drawings of the project (aaach additional pages ijnecessary): Per attached backup material this Change Order #2 includes the follmving items: Item 1) Provide & install gate valves on the PW and NPW lines to the Headworks Odor Control Area at a cost of $1,829. Item 2) Required modifications to the 10" Supernatant Line at a cost of $5.398. Item 3) 100' extension to 6-inch PW line to tie in location per RFl 41 at a cost of $10.386. Item 4) Provide & install additional 6" glass -lined ductile iron pipe fittings per RFI 7 at a cost of $8,670. Item 5) Extend 4" Centrifuge drain to manhole at a cost of $11.488. Item 6) Providing & installing 8" sludge pump suction line in Tank 7 to replace the line damaged from upset cover at a cost of $11,328. Item 7) Additional 6" pipe & fittings at the DSPS sump pump piping, and 6" CPVC ball valve in the polymer room at a cost of $9.543. Item 8) Work & materials for a new construction joint in the conveyor pit at a cost of $21.510. Item 9) Modifications to the roof of the existing control room between Sludge Holding Tanks 6 & 7 per PCM 02 at a cost of $30,308, and 15 additional days. Item 10) 30 additional days due to changes and clarifications that have impacted the project schedule. Where the Change Order is negotiated. the Change Order must be fully documented and itemized as to costs, including material quantities, material costs, taxes, insurance, employee benefits, other related costs, profit and overhead. Where certain unit prices are contained in the initial contract, no deviations are allowed in computing negotiated change order costs. Senior Engineer City of Lubbock, Texas July 12, 2016 Page 2 Item 1 Archer Western is requesting $1,829 for additional work required to install four 6" and 8" gate valves on the potable water (PW) and non -potable water (NPW) lines associated with service to the new odor control at the Headworks. The lines were shown, but the valves were omitted from the plans. The gate valves provide needed utility isolation. The Engineer and RPR have reviewed the request, and have found the cost and effort to be in line with what would be expected. The following documents are attached to support Item 1: 1. Potential Contract Modification (PCM) #12 dated April 19, 2016 from Jason Buoy of Archer Western with the cost backup for the additional work. 2. PCM #12 Response from Jason Anderson of Carollo Engineers with the Engineer's response to the proposal. Item 2 40 feet of a 10" supernatant line that was intended to be reused was found to be corroded and unusable upon excavation. Archer Western submitted a cost proposal for $5,398 replacement of the corroded pipline. The Engineer and RPR have reviewed the request, and have found the cost and effort to be in line with what would be expected for this unforeseen condition. The Engineer recommends acceptance of this item. The following documents are attached to support Item 2: 1. Potential Contract Modification (PCM) #13 dated April 15, 2016 from Jason Buoy of Archer Western with the cost backup for the additional work. 2. PCM #13 Response from Jason Anderson of Carollo Engineers with the Engineer's response to the proposal. Item 3 Archer Western is requesting $10,386 for additional work related to RFI #41 to extend the new 6" potable water (PW) line serving the Solids Handling Building an additional 100' further than indicated on Drawing 0OC13 to connect to an existing 6" PW line. The 6" PW line was shown on Drawing 0OC13 to connect to an existing line at Pipe Coordinate V. Once uncovered, the Contractor found the existing PW line at this location to be a 3" copper line, which was different than what was shown on the Record Drawings. Through further investigation the Contractor found that the 6" PW line intended for connection was located along the road on the north side of the Maintenance Building. To be able to service the Solids Handling Building with this 6" PW line the connection point would need to move to the north of Pipe Coordinate #7 by approximately 100 feet. The Engineer agreed in the response to RR #41 that the additional piping would be necessary. Item 4 Archer Western is requesting $8,670 for additional work related to RFI #7 to provide and install additional 6" glass -lined ductile iron pipe fittings associated with reconfiguration of the underground digested sludge (DS) piping at Sludge Holding Tanks 6 and 7. In RFI #7 the Contractor expressed concern that the four DS lines between the Digested Sludge Pump Station and the Solids Handling Building would have to cross each other to connect DS Pump #1 with Centrifuge #1, DS Pump #2 with Centrifuge #2, and DS Pump #3 with Centrifuge #3. The problem was created because the Record Drawings for the existing DS piping did not reflect the actual field conditions. The Engineer provided modified yard piping drawings to establish the desired piping arrangement that would result in the correct pump being associated with the correct centrifuge. The new piping arrangement required additional 6" glass -lined ductile iron pipe fittings. The Engineer and RPR have reviewed the request, and have found the cost and effort to be in line with what would be expected. The following documents are attached to support Item 4: 1. Item 5 Archer Western is requesting $11,488 for additional work associated with installation of new 4" drain piping in the Solids Handling Building from sample sinks to an existing manhole. This work was required because the original drain piping within the slab was found to have corroded away, leaving only the concrete encasement to carry water. Item 6 Archer Western is requesting $11,328 for additional work associated with providing and installing a new 8" sludge pump suction line inside Sludge Holding Tank 7. This work was required because the original floating digester cover had tilted and jammed, and in the process the 12' deep steel skirt had damaged the original 8" sludge pump suction line and pipe supports. The Engineer and RPR have reviewed the request, and have found the cost and effort to be in line with what would be expected in response to this unforeseen condition. Item 7 Archer Western is requesting $9,543 for additional work associated with providing and installing additional 6-inch pipe and fittings to connect the Digested Sludge Pump Station sump pump piping to manhole. This was required because the existing line to the manhole was found to be only 2-inch diameter. Additional 6" CPVC Ball Valve used in the polymer room is also included on this PCM. This additional valve will ease construction phasing for the new polymer system. The Engineer and RPR have reviewed the request, and have found the cost and effort to be in line with what would be expected. The following documents are attached to support Item 7: 1. Senior Engineer City of Lubbock, Texas July 12, 2016 Page 5 Item 8 Archer Western is requesting $21,510 for additional work and materials needed to create a new construction joint to allow staged construction of the conveyor pit. This approach was required to allow City personnel continuous access to the center sludge truck loading bay of the Solids Handling Building. The cost includes $6,803 for a steel road plate, $2,400 for forms, 15 yards of flowable fill at $1,305, 4" SDR piping at $328, beams at $522, 195 hours of labor for $3,900 and the Contractor's labor burden at $2,262, and small tools at $390 for a total direct cost of $18,011. Overhead and profit, subsistence, and insurance and bonds total $3,499. The Engineer and RPR have reviewed the request, and have found the cost and effort to be in line with what would be expected. Item 9 Archer Western is requesting $30,308 for work involved to pour 12-inch wide, 24-inch high beams on the roof of the existing control room between Sludge Holding Tanks 6 and 7 to perform work outlined in PCM 02 at this area. This structural modification was required to allow for the opening in the roof necessary to install the new sludge holding tank overflow piping. The Engineer and RPR have reviewed the request, and have found the cost and effort to be in line with what would be expected. The Contractor has not requested a time extension with this PCM #22, but does request an extension of 30 days for the work performed under this PCM #22. The Engineer and RPR have recommended an extension of 15 days for this work. The following documents are attached to support Item 9: 1. Item 10 Archer Western has requested 60 additional days due to changes and clarifications that have impacted the project schedule. This includes work related to RFI's 71, 78, 80, 82, 83, & 85; DC- 6 (all electrical work), and work associated with the roof modifications at the existing Solids Handling Building, and roof modifications at the Sludge Holding Tank Control Room that has 8900A.301 Fekele00T.dwx carol lo.com Mr. Zoltan Fekete, P.E. The Engineer and RPR have reviewed the request, and have found the time requested to be in line with what would be expected, except for the roof modifications, in which case the recommendation is 15 days, for a total of 45 days added to the Contract.
