Passed by the City Council on July 28.2016 DANIEL M.POPS.MAYOR ATTEST: Li^ Rebecca Garza.City Secreta APPROVED AS TO CONTENT: Leisa Hutcheson,Director of Human Resources & Risk Management APPROVED AS TOJIQRM:XEQaM: itcheir^^yvjpite,First Assistant City Attorney RES.Agrmt-Amend -UMC Physicians Network 5.25.16 Resolution No. 2016-RO243 AMENDMENT AGREEMENT FOR SERVICES City of Lubbock Employee Medical Clinic Services This Amendment applies to an Agreement previously entered in between UMC Physicians, formerly known as UMC Physician Network Services ("UMCP") and the City of Lubbock ("the City"), Resolution Number 2013-110427, effective February 1, 2014. This Agreement and this Amendment set forth all terms, conditions, and requirements for the parties and supersede any earlier agreements, writings, or promises. In the event of conflict or inconsistency between the Agreement and this Amendment, the terms of this Amendment shall govern. The parties agree to amend the Agreement as follows, effective August 1, 2016: Replace all references in the document to "UMC Physician Network Services" and "PNS" with "UMC Physicians" and "UMCP." Delete Paragraph 2.2.2, "Health Risk Assessment," in its entirety. Delete and replace Paragraph 2.2.3, "Reporting," with the following: 2.2.3 Reporting. UMCP will provide to the City, no later than the 15' day of the month immediately following the end of each month of the calendar year, a written report with respect to the provision of the medical services during the immediately preceding month. The written report shall be in a form reasonably satisfactory to the City and shall specifically indicate (a) the number of covered Plan Members seen with diagnosis codes and (b) savings report in a form to be agreed upon by UMCP and the City. Delete the following from Paragraph 3.1, "Fees": Health Risk Assessment — $105.00 per Health Risk Assessment (screeningand questionnaire) billed separately from medical services. All other terms and conditions of the Agreement shall continue in full force and effect without change. This contract amendment is subject to the City of Lubbock's Non -Arbitration clause listed in "Exhibit B". CITY OF LUBBO K By: / Daniel M.
