RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF~THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock BE and is hereby authorized and directed to
execute for and on behalf of the City of Lubbock an Amendment to the Lease Agreement
between the City of Lubbock and Community Health Center of Lubbock. Said Amendment is
attached hereto and incorporated in this Resolution as if fully set forth herein and shall be
included in the minutes of the City Council.
Passed by the City Council this 26th day of June 2001.

AMENDMENT Item No. 36
This Amendment to the Agreement or Lease is entered into by and between the
City of Lubbock, a Texas home rule municipal corporation hereinafter referred to as the
"Lessor" and Community Health Center of Lubbock, hereinafter referred to as "Lessee,"
and collectively referred to as the "Parties."
WITNESSETH:
WHEREAS, the Lessor owns certain buildings and a tract of land at 1301
Broadway, Lubbock County, Texas, and more particularly described as all of Block 130,
Original Town Addition to the City of Lubbock, Lubbock County, Texas;
WHEREAS, the Lessor and Lessee have entered into an agreement executed
December 16, 1999 (Resolution No. 663 8) (the "Agreement" or "Lease"); and
WHEREAS, it is the desire of the Lessee to lease additional space from Lessor
and the desire of Lessor to lease additional space to Lessee and thus both parties desire to
amend the terms of the Agreement by amending and/or adding certain sections of the
Agreement;
NOW THEREFORE, for and in consideration of the covenants and conditions
herein contained and other valuable consideration:
1. Lessor agrees to add to the leased premises of Lessee the area commonly
referred to as Suite 6. Exhibit "A" of the Agreement executed December 16, 1999
(Resolution No. 6638) is hereby deleted in its entirety and replaced in its entirety with
Exhibit "Al" which is attached hereto and incorporated herein as though set forth fully
herein.
2. The remaining terms and conditions of the Agreement remain in full force
and effect and the Parties are not amending in any way manner or form the remaining
terms and conditions (save and except those amendments outlined in Section 1 above) of
the Agreement.
