RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock BE and is hereby authorized and
directed to execute for and on behalf of the City of Lubbock, an Amendment to the
Employee Benefit Plan in regard to treatment for Autism Spectrum Disorder, and related
documents, and shall submit said Amendment and documents, if any, to the City's third
party administrator for use in determining and administering employee benefits. Said
Amendment is attached hereto and incorporated in this resolution as if fully set forth
herein and shall be included in the minutes of the City Council.
Passed by the City Council this 14th day of January , 2010.

Employee Benefit Plan
Plan Amendment No. 2
This Amendment to the City of Lubbock employee Benefit Plan Document (the "Plan
Document") is executed on this, the 14th day of January, 2010, to be effective January
1, 2010.
All capitalized terms shall have the same meaning as those terms are defined in the Plan
Document. The following change is hereby made to the Plan Document:
• Benefits for Treatment of Autism Spectrum Disorder will be available for a
covered dependent child from birth to age 9 subject to deductibles, copayments
and co-insurance.
