VITAL STATISTICS.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF
Section 1. That all physicians, surgeons or accouc eurs m wives)
who may attend at the birth of a child in this city, or in the absence
of such attendance the father of such child or if such child have no
father then its mother shall report the'fact together with name, sex,
color and such other data relating thereto as may be prescribed by
the rules promulgated by the State Board of Health within five days
from the timo of the birth to the city pecrotary as hereinafter pro-
v1ded for. 'Any physician, surgson or accoucheur or parent who shall
fail to make the report above`provided shall be punished by fine of
one urvcired dr%
Section 2. Every person acting as under -taker ahal H1e with the
city secretary a certificate of death and obtain a burial permit from
the city secretary in each case (including stillbirths) in which ha so
acts, and all persons furnishing a coffin or box in which to bury the
dead shall be included within the term undertakers. It shall be the
duty of such undertaker to accurately and properly fill out the death
certificate in so far as regards the name, a§e, sex, nativity and such
other "personal and statistical particulars, and further, he shall
obtain from the physician or o�rcnor the answers to questions under the
heading of "medical particulars" of the death certificate. Any undert
taker who shall fail to file the certificate of death above previ.ded
or who shall bury the body of any deceased person without first obtain-
ing & burial permit from the city secretary, shall be finod in any sum
not less than five dollars, nor more than one hundrod dollars.
Section 3. The body of any person whoso death or burial occurs
in this city shall not be interred, deposited in a vault or tomb, cre-
mated or otherwise disposed of or removed or transported from this city
until a permit for burial, removal or other disposition shall have been
issued by the city secretary, and no such burial or removal permit
shall be issued by the city secretary until a complete certificate of
the death has been filed with him as hereinbofore provided: Provided,
that a transit pormit issued in accordance with the law and health
regulations cLf --
the city secretary as a basis upon which RP Shai issue a'"6ur'Ia
permit. Any person who shall cause or permit to bn interred, crAmated,
deposited in a vault or tomb, or otherwise disposed of or removed or
transported out of the city the remains of any deceased person with-
out having first obtained the permit above provided for from the city
secretary, shall be fined in any sum not less than five dollars, nor
more than one hundred dolLnrs.
Section 4. It shall be t:_s duty of every physician in the event
of a death (including stillbirths) occurring in any case at which said
physician is the last in attendance, (or in the event a coroner shall
held an inquest to ascertain the cause of death, the said coroner's
duty) to promptly and accurately fill out the question in the form of
the death certificate when the death certificate is presented by the
undertaker. Any physician or coroner who shall fail or refuse to
promptly and accurately fill out the questions in the death certificate
when requested by the undertaker shall be fined in any sum not less
than five dollars, nor more than one hundred dollars.
Section 5. In the event of a death ^ccurring without a phyx:i-
cian in attendance and the services of a coroner not necessary, it
then shall become the duty of the city health officer to investigate
the case and to issue the certificate of death.
Section 6. All certificates of births and deaths shall be made in
the manner prescribed, and on the form of certificate as supplied by
the State Board of Health.
Section 7. It shall be the duty of the aforomentioned city sec-
retary to record in a permanently bound book all births and deaths
which are reported to him together with such statistics and data which
shall be furnished him by the birth certificate and death certificato.
And it shall be the duty of said city secretary to transmit all such
original birth and death certificates received during the preceding
month to the State Registrar of Vital Statistics at Austin, Toxas, on
or before the loth of the following month.
Section 8. That this ordinance shall take effect and be in force
from and after its passage.
