RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute for
and on behalf of the City of Lubbock, Amendment No. 1 to Contract No. 14321 dated November
15, 2018, for veterinary services, by and between the City of Lubbock and Eila Machado,
D.V.M., and related documents. Said Amendment is attached hereto and incorporated in this
resolution as if fully set forth herein and shall be included in the minutes of the City Council.

AMENDMENT NO. 1
TO AGREEMENT BETWEEN TI-CE CITY OF LUBBOCK
AND
Eila Machado, D.V.M. .
TIIIS IS AN AMENDMENT TO THE AGREEMENT 14321 dated and entered into the 15th day of November
2018 by and between the City of Lubbock ("City") and Eila Machado, D.V.M.. ("Contractor") of Lubbock, TX.
The Contractor has entered into the Agreement with the City to provide Veterinary Service.
Pursuit to the Contractors response in"Exceptions to the specification', the City desires the Contractor to provide
additional services. This Amendment No. 1 adds Sterilization of Animals at Lubbock Animal Shelter Adoption
Center per Exhibit "A", attached hereto, to the Scope of Services to be performed by Contractor
All other portions of the original Agreement and previous Amendments if any, shall remain in place and are not
altered by this amendment.

Price Proposal Addendum to Contract RFP 18-14321- SG; Veterinary Services
Surgical Services: Sterilization of Animals at Lubbock Animal Shelter Adoption Center
Summary: Sterilization surgeries will be perfonned every Tuesday and Thursday between the
hours od l0am and 2pm after which routine medical rounds of the facility will be perfonned.
My surgical duties will include:
• Pre-surgical physical exams of patients
• Measurement of hematocrit, packed cell volume, blood glucose, and fecal parasite
load if deemed appropriate based on physical exam findings
• Pre-surgical sedation administration
• Induction and intubation
• Perform ovariohysterectomy(OVH) on female patients and/or scrotal or
prescrotal orchiectomy(castration) on male patients.
• Extubate, and provide appropriate warning measures to animals recovering from
anesthesia.
• Post-operative monitoring of patients and administration of appropriate post-
operative pain control
I propose an hourly rate of$75/hour for these services
Approximate weekly cost: $600
Approximate yearly cost: $31,200
The surgical program's success should be re-evaluated by the end of the 2019 fiscal year(at the
latest) at which time additional surgery days, alternate surgical patient profiles, and the addition
of a part time or full time veterinary technician should be addressed.
