Said Change Order No. 02 is attached hereto and incorporated in this resolution as fully set forth herein and shall be included in the minutes of the City Council. Contract Title: 1141 St Paving Improvements Slide Rd to Quaker Ave Project No: 92669.9240.30000 & 92750.9241.30000 "Change Order" means a written order to a Contractor, executed by the Owner, in accordance with the Contract authorizing an addition to, deletion from, or adjustment or revision of the requirements of the Contract documents, or an adjustment to the compensation payable to the Contractor, or to the time for performance of the Contract and completion of the project, or a combination thereof, which does not alter the nature of project and is an integral part of the project objective. Adjustments to "Estimated Quantities" to a line time in a Unit Price Contract do not require a Change Order. All work that alters the nature of the construction or that is not an integral part of the project objective must be let out for public bid. Description of Change (alteration, deviation, addition, or deletion) caused by conditions encountered during construction not covered by the s ecifiications and drawings of the project Jattached additional pages is necessary: Change Order 02 to Contrad 16114 includes the relocation of approximately 1500 linear feet of 12" C900 PVC line and 100 linear feet of 8" C900 PVC. The location of the existing line is in conflict with the proposed drainage channel for 1141 Street from Slide Road to Quaker Avenue. This Change Order will result in an additional $284,562.00 to the Contract amount and an additional 60 days of construction time. Where the Change Order is negotiated, the Change Order must be fully documented and itemized as to costs, including material quantities, material costs, taxes, insurance, employee benefits, other related costs, profit and overhead. Where certain unit prices are contained in the initial Contract, no deviations are allowed in computing negotiated change order costs. I have attached a unit price breakdown of the work based upon the units we received in your March 3, 2023 email and these prices cover: materials, installation, performance bond, and markup. Please note that in addition to the unit prices attached, LSDP requests that an additional 60 days be added to the contract for the preparation and performance of this scope of work. LSDP will remain onsite and continue to perform any work activities that do not directly interfere with with the progress of the 12" water line relocation whether or not these activitites are on the critical path. However, should we reach a point where we have not received approval of this change order and we run out of productive work activities LSDP requests that time be suspended or days be credited until such approval is obtained.
