BACKGROUND The social determinants of health (SDoH), as defined by the Centers for Disease Control (CDC), "are the conditions in the places where people live, learn, work, and play that affect a wide range of health and quality -of -life -risks and outcomes." The CDC and Healthy People 2030 classify the social determinants of health into 5 domains: Economic Stability, Education Access and Quality, Health Care Access and Quality, Neighborhood and Built Environment, and Social and Community Context. These domains include items such as racism and discrimination, access to nutritious foods, availability of physical activity, literacy, and access to transportation. Aside from having a large impact on people's health and well-being, they contribute to health disparities and inequalities, and can account for 80% of a person's health. The impact of SDoH on health care utilization, cost and outcomes is well documented. As the public health environment evolves, addressing SDoH, or the nonmedical drivers that impact a person's health, will continue to drive a host of evolving policies, initiatives, practices and partnerships. As healthcare transitions from volume to value -based care, health departments need to demonstrate the high quality of public healthcare, facilitate improved outcomes for high -risk and vulnerable populations, and effectively shift to whole -person care. In order to do so, health departments will need to develop the tools necessary for leveraging social determinant data and addressing them with their patient populations. To date, LBK Community has served over 400 individuals across Lubbock, Texas since June 2020. LBK Community serves medically and socially underserved areas and populations; approximately 446 referrals have been made with 70% of said referrals closed or completed. Moreover, 771 needs have been identified with 64% of those needs addressed. The Lubbock Health Department is committed to providing programs and services that will strengthen and improve health outcomes across the city. This RFP seeks services to assist the Lubbock Health Department in screening clients for Social Determinants of Health as well as referring at -risk clients to appropriate social services. The desired system would have: • Social need screening and referral capabilities. • Referral tracking capabilities. • A platform that may be utilized directly by a provider, client and community partner. • Platform with flexible architecture to allows for the integration of multiple interfaces and technology systems. • A commitment to development of CBO partnerships in Lubbock. • A quality assurance'CBO verification process. Support for bidirectional Electronic Health Record (EHR) integration and Application Programming Interface (API) development. • Portals and/or Dashboard reports related to resource need, utilization, referral loop closure, patient type, and related health outcomes. • Health Information Portability and Accountability Act (HIIP AA) compliant systems that meet all Federal and State Privacy and security and data protection requirements. 2. SCOPE OF WORK The selected vendor will provide Social Determinant of Health Collection and Referral services to LBK Community. The selected vendor will have a robust network of participating Community Based Organizations within a centralized database to help identify and meet the social needs of LBK Community clients. The platform should allow LBK partners to directly screen clients, as well as provide clients the ability to initiate the screening and referral process. As the Lubbock Health Department has long-standing community relationships, the selected vendor will work with users to update their centralized database to include preferred community partners, as well as update the active status of partners based on response rates. Data will be provided to the LBK Community partners related to commonly screened social needs, referral tracking and associated health outcomes. Selected vendor (s) may offer additional services for individual LBK partner CBO's. These other services can include but are not limited to populations health management, data analysis and data visualization services. 3. DETAILS AND QUESTIONNAIRE 3.1.
