MCMI ,MAYOR Resolution #2878 AMENDMENT NO. 1 to RESOLUTION #2607 FOR EMERGENCY SHELTER FUNDING AGREEMENT BETWEEN THE CITY OF LUBBOCK AND GUADALUPE ECONOMIC SERVICES CORPORATION THE STATE OF TEXAS § COUNTY OF LUBBOCK § The Amendment to an Agreement is entered into this 14th day of July, 1988, between the City of Lubbock, Texas, a home rule municipal corporation, hereinafter called "City" and Guadalupe Economic Services Corporation, a nonprofit corporation incorporated under the laws of the State of Texas, hereinafter called "Corporation." WITNESSETH: SECTION 1. AGREEMENT PERIOD The parties hereto agree to amend Section 1. Agreement Period of the Funding Agreement to extend the termination date from the originally established date of June 24, 1988, to the amended termination date of August 28, 1988. SECTION 2. The parties hereto agree to amend Exhibit A, Performance Statement, of the Funding Agreement by deleting it in its entirety and substituting in lieu thereof, Exhibit A, Performance Statement, which is attached hereto and hereby made a part of this amendment. SECTION 3. The parties hereto agree to amend Exhibit B, Budget, of the Funding Agreement by deleting it in its entirety and substituting in lieu thereof Exhibit B, Budget, of this amendment, which is attached hereto and hereby made a part of this amendment. SECTION 4. The parties hereto agree that all terms of the above named funding agreement and all amendments thereof executed prior to the effective date of this amendment shall remain in effect and shall continue to govern except to the extent that they conflict with this amendment. SECTION 5. The amendment is not effective until approved and accepted by the Texas Department of Community Affairs. Brewster ivil Trial Attorney EXHIBIT A PERFORMANCE STATEMENT GUADALUPE ECONOMIC SERVICES CORPORATION Section 1. Renovation, Major Rehabilitation/Conversion A grant shall be awarded to the Guadalupe Economic Services Corporation for the sum of Twenty Five Thousand Fifty Three and no/100 Dollars ($25,053.00) to rehabilitate a building at 2701 Clovis Road into five units for homeless families, and convert a second building into a food pantry and meeting room for client -agency contact and training and repair parking area. Matching funds of Eight Thousand One Hundred Sixty Seven and no/100 Dollars ($8,167.00) shall be from the value of the lease on the building at 2701 Clovis Road; Five Thousand Seven Hundred Ten and no/100 Dollars ($5,710.00) from donated labor; and Seven Thousand Five Hundred Seventy Six and no/100 Dollars ($7,576.00) from services contributed by volunteers; One Thousand Five Hundred and no/100 Dollars ($1,500.00) from donated food; and Two Thousand One Hundred and no/Dollars ($2,100.00) from the value of donated building supplies. Section 2. Provision of Essential Services City shall provide a grant to the Guadalupe Economic Services Corporation for the sum of Fifty Four and no/100 Dollars ($54.00) for the provision of health and employment services, food, and transportation. Matching funds of Fifty Four and no/100 Dollars ($54.00) shall be from the value of time and services contributed by volunteers. Section 3. Payment of maintenance, operation (including rent, but excluding staff), insurance, utilities, and furnishings City shall provide a grant to the Guadalupe Economic Services Corporation for the sum of One Thousand Nine Hundred Seventy One and no/100 Dollars ($1,971.00) of contract funds for the payment of cost of utilities and insurance at Five Hundred Twenty Five and no/100 Dollars ($525.00) per month for a period of three and three-quarters (3.75) months. Matching funds of One Thousand Nine Hundred Seventy One and no/100 Dollars ($1,971.00) shall be from the value of time and services contributed by volunteers. Also - City shall provide a grant to the Guadalupe Economic Services Corporation services for the sum of Two Thousand Nine Hundred Twenty Two and no/100 Dollars ($2,922.00) to purchase necessary furnishings for the renovated facility. Matching funds of Five Hundred Eighty Five and no/100 Dollars ($585.00) shall be from private donations and Two Thousand Three Hundred Thirty Seven and no/100 Dollars ($2,337.00) shall be from the value of time and services contributed by volunteers. 2 EXHIBIT B GUADALUPE ECONOMIC SERVICES CORPORATION BUDGET Category Contract Funds Match Funds Total Source of Match 1.
