RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
"THAT Amendment No. 2 to the City of Lubbock Master Welfare Benefit Plan regarding
Nondiscrimination in Health Programs and Activities is hereby adopted and approved, and that
the Mayor or City Manager, as necessary. is authorized and directed to execute any documents
necessary to effectuate said Amendment No. 2 or its purposes. Said Amendment is attached
hereto and incorporated in this resolution as if fully set forth herein, and shall be included in the
minutes of the City Council,"

Employee Benefit Plan
Plan Amendment No. 2
This Amendment to the City of Lubbock employee Benefit Plan Document (the "Master
Welfare Benefit Plan and Summary Plan Description") is executed on this, the day
ol'October, 2016 to be effective .January 1, 2016.
Any blanket exclusions regarding medically necessary gender transition services
are removed to comply with Section 1557 Nondiscrimination in Health Programs
and Activities as required under the Affordable Care Act (ACA). Services shall
be covered as any other benefit and will be subject to the Medical Policy of Blue
Cross Blue Shield of Texas, the City of Lubbock's administrative services only
provider.
