Said Amendment is attached hereto and incorporated in this resolution as if fully set forth herein and shall be included in the minutes of the City Council. Effective Date The initial Effective Date of the Plan was January 1, 2004. Plan Administrator Lee Ann Dumbauld, City Manager and Named City of Lubbock Fiduciary 1625 13`h Street Lubbock, Texas 79457 The Plan Administrator is responsible for maintaining records on Participants, determining eligibility for benefits, and interpreting and administering the provisions of the Plan.
