Cook As Health Authority - 12/15/2010Resolution No. 2010-RO627 December 15, 2010 Item No. 5.40 RESOLUTION WHEREAS, the City Council of the City of Lubbock has determined that it is in the best interest of the citizens of the City of Lubbock to appoint the following named person as the Local Health Authority for the City of Lubbock, Texas, pursuant to Chapter 121 of the Health and Safety Code; NOW THEREFORE: BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK: SECTION 1. THAT the following named person is hereby appointed Local Health Authority for the City of Lubbock, Texas. Ronald L. Cook, DO, MBA SECTION 2. THAT said Local Health Authority shall serve for a term of two (2) years or until such time as a successor has been appointed. SECTION 3. THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute a Certificate of Appointment for a Local Health Authority and any associated documents on behalf of Dr. Cook. Date term of office ends December 31, 2012, unless removed by law. The Local Health Authority has been appointed and approved by the City Council for the City of Lubbock, Texas.
