Adjustments to "Estimated Quantities" to a line item in a Unit Price contract do not require a Change Order. All work that alters the nature of the thing to be constructed or that is not an integral part of the project objective must be let out for public bid. Description of change (alteration, deviation, addition, or deletion) caused by conditions encountered during construction not covered by the specifications and drawings of the project (attach additional pages if necessary): This change order is to increase the amount of micro -surfacing that is being placed in designated locations. The City of Lubbock standard specifications reads that the micro -surfacing shall be placed at a minimum of 23 pounds per square yard and a maximum of 30 pounds per square yard. At the pre-bid meeting that was held on March 10, 2010, it was discussed what locations were going to need additional amounts of micro -surfacing from 23 pounds per square yard to 25 pounds per square yard. Following the pre-bid meeting, no addendum to the specifications was sent out clarifying those changes. The contrcator is currently placing the minimum amount of 23 pounds per sqaure yard. In order to properly cover some of the areas that we have left on the contract, we need to increase the amount being placed from a minimum of 23 pounds per square yard to a minimum of 25 pounds per square yard. Which will result in a $0.20 a square yard increase in price. Where the Change Order is negotiated, the Change Order must be fully documented and itemized as to costs, including material quantities, material costs, taxes, insurance, employee benefits, other related costs, profit and overhead. Where certain unit prices are contained in the initial contract, no deviations are allowed in computing negotiated change order costs. ITEMIZED COSTS MUST BE FULLY DOCUMENTED AND ATTACHED TO THIS FORM. ITEM DESCRIPTION AMOUNT A. ORIGINAL CONTRACT VALUE: $3,441,017 B.
