RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute for
and on behalf of the City of Lubbock, Contract Number 16-12662-JOC for building envelope
repair project at City Hall, 1625 131h Street, by and between the City of Lubbock and Collier
Construction Company, of Lubbock. Texas, and related documents. Said Contract is attached
hereto and incorporated in this resolution as 11' fully set forth herein and shall be included in the
minutes of the City Council.

The following items detail the scope of work as discussed at the site. All requirements necessary to accomplish the items
set forth below shall be considered part of this scope of work.
1. Work shall include removal of existing concrete walkways along the west, south, and north sides of the facility. as
described in the project drawings in order to install new concrete sloped walks and curbing along the base of the existing
building. 2. Work shall include the removal of the existing wall finish and metal stud framing from below the existing
window units and existing aluminum reveal along the entire length of the west south and partial north face of the building as
described in the project drawings, in order to install new cold form metal framing, insulation, exterior sheathing,
damproofing and new three -fourth inch (314") PC Plaster. Contractor shall provide a five (5) year labor and materials
warranty and a five (5) year manufacturers material warranty. 3. Work shall include removal of existing sealant at all glass
to metal joints on all exterior elevations as described in the project specifications. Contractor shall install all specified
sealant and required accessories as per the project specifications. Contractor shall provide Owner with a five (5) year labor
and materials warranty and a five (5) year manufacturers material warranty. 4. Work shall include removal of the existing
light poles, and exit signs, along Paris Ave. and 13th Street, all associated conduit and electrical wiring back to the
existing junction box on the south west corner of the building as described in the project drawings. Contractor shall clean
and reinstall light poles and exit signs (where shown), install all new underground electrical conduit and electrical wiring
from the existing junction box out to the light pole locations and make all necessary hook ups to maintain fully functional
light poles with all required accessories as per the project drawings. Contractor shah provide Owner with a rive (5) year
labor and materials warranty on all work. 5. Work shall include the infill of the designated underground vaults and
encapsulating the existing electrical conduit in order to install new concrete walkways as described in the project
drawings. 6. Work shall include extending the tops of the existing manhole and adjacent rectangular vault and providing a
new watertight manhole cover and new watertight hinged vault door up to the new concrete walkway as described in the
project drawings. 7. Work shall include removal of a portion of the existing retaining concrete wall along the existing
loading dock drive location and installing a new concrete retaining wall as described in the project drawings. 8. Work shall
include removing the existing exit only, door on the west side of the building on Paris Ave. and replacing with a new 16 ga.
Hollow Metal Door with all associated hardware, reconnect the existing building security as required, provide new painted
finish on both faces of the door and frame, rework the existing wall finishes inside adjacent to the door back to its original
condition as described in the project drawings. Contractor shall provide Owner with a five (5) year labor and materiels
warranty on all work and a five (5) year manufacturers material warranty.

Dear Scott,
It is our pleasure to present to you our written quotation on the above referenced project.
This work is as we discussed in our job site meeting. Scope of work is as follows:
1) Remove and replace sidewalk on three sides of building.
2) At same elevations, remove up to 5 feet of plaster. Removal
is up to metal reglet. Once side work is removed, dig
approximately 1 foot of soil at sidewalk perimeter Damp-proof
wall, apply waterproofing a foot below sidewalk and then thru
wall flashing. We will then place new standard plaster (not
marble type). Caulk new plaster as well as sidewalk to piaster,
three sides of building
3) Wet glaze windows on all 3 sides of building
4) Re -caulk window perimeters on all four sides of building.
5) Replace low retaining wall.
6) Lift utility covers that are not being replaced.
7) Create curb on three sides of building (excludes handicap ramp),
8) Work door and frame as specified.
9) Caulk side to curb and sidewalk to sidewalk.
Exclusions:
1) Any and all abatements.
2) All permits, etc.
3) Payment and performance bond.
4) Work related to back up wall.
5) Work related to filling utility holes.
6) Water that enters through hollow metal doors and its elements.
7) Water that penetrates above our waterproof membrane, also below.
8) All electrical, plumbing, etc.
9) Utilities and facilities

11) Any and all work related to repairs to work on exteriorfarade
above our work.
