RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock is hereby authorized and directed to
execute for and on behalf of the City of Lubbock a Contract Change No. 001 A for CHS-
Population Based Services (DSHS Document No. 2007-02041 l) and any associated
documents by and between the City of Lubbock and the State Department of State Health
Services, a copy of which Contract Change and any associated documents are attached
hereto and which shall be spread upon the minutes of this Council and as spread upon the
minutes of this Council shall constitute and be a part hereof as if fully copied herein in
detail.

AGENDA ITEM SUMMARY
ITEM # / SUMMARY:
Consider a resolution authorizing and directing the Mayor to execute for and on behalf of the
City of Lubbock, a Change No. 001A to the DSHS Contract No. 2007-020411 CHS-Population
Based Services and any associated documents, by and between the City of Lubbock and the
Texas Department of State Health Services.
BACKGROUND/DISCUSSION:
The CHS-Population Based Services contract was originally approved by Council with
Resolution # 2006-RO400 on August 24, 2006.
The current Contract Change Notice No. 001A to DSHS Document No. 2007-020411 reduces
approved funding from $107,414.00 to total payments not exceeding $3,500. This is due to
delays in hiring a Health Education Coordinator to fully implement the program. The FY 06-
07 is now considered a planning year and full funding is expected to be executed and
restored with the FY 07-08 contract for the actual implementation of the program objectives.
The $3,500 will cover expenses for identifying and validating the curriculum for parent
education to reduce teen pregnancy and STDs, formation and meetings for the teen
pregnancy and sexually transmitted disease (STD) coalition, and assuring that the project has
adequate sample size.
FISCAL IMPACT:
There is no impact to the general fund.
