Vfiperwo4 Reduction Act Statement: This form is the FAA's primary source for collecting information for the authority to collect PFC revenue for
li�irport d elopment. This information is used to determine the eligibility and justification of airport development projects regarding safety, security,
o city of the national air transportation system; or which reduce noise or mitigate noise impacts resulting from an airport; or fumish
opportunities for enhanced competition between or among air carriers. It is estimated that it will take approximately 5-80 hours to fill out the
application depending on the complexity. The use of the form is required to obtain FAA approval of authority to collect PFC revenue (49 U.S.C.
40117(c)). No assurance of confidentiality is necessary or provided. It should be noted that an agency may not conduct or sponsor, and a person is
not required to respond to a collection of information unless it displays a currently valid OMB control number. The OMB control number associated
with this collection of information is 2120-0557.

ATTACHMENT B — PROJECT INFORMATION
PFC APPLICATION NO.04-05-C-00-LBB AMENDMENT
In accordance with 14 CFR Part 158 section 158.37, the City of Lubbock, Texas is requesting authorization to
amend the existing FAA approved impose and use PFC Application No. 04-05-C-01-LBB for the following
project:
Item 5-5, Upgrade Access Control/CCTV - Amend PFC collection amount due to increased
construction costs.
Justification — During the design phase, it was found that additional equipment for
the access control and CCTV systems was needed to provide the proper
communications between access points and the central control office. This
included software and hardware necessary for secure transmission of data and fail -
over control. Additionally, it was determined that additional CCTV cameras were
needed for the exterior of the terminal building to provide adequate visual control
of access points onto the airport operations area. These changes along with
increased construction costs account for the difference in the proposed PFC budget
and the actual bid amount.

The original application Record of Decision Date is October 21, 2004. The current Charge Expiration Date for
this application is February 1, 2007. The amended estimated Charge Expiration Date is August 1, 2007.
This amendment will result in a net increase in PFC collection amount for this application. See attached
Table B-1.

Consider a resolution authorizing the Mayor to approve the amendment to the
Passenger Facility charge Application 04-05-C-01-LBB.
Item Summarl,
PFC Application 04-05-C-01-LBB will be amended to reflect budget changes to
project 5-5, Upgrade Access Control/CCTV project. The amendment increases the
Impose and Use Authority by $517,000 from $4,763,392 to $5,280,392,
The action to amend the PFC collection amount is due to higher than anticipated
construction costs.
