Said Change Order No. 9 is attached hereto and incorporated in this resolution as if fully set forth herein and shall be included in the minutes of the City Council. Date: December 22, 2009 BID/ITB #: I 07-730-BM Contract #: I 81 19 I Project Name: I Southeast Water Reclamation Plant Improvement Change Order within the scope of the contract means a Change Order that DOES NOT ALTER THE NATURE OF THE THING TO BE CONSTRUCTED AND THAT IS AN INTEGRAL PART OF THE PROJECT OBJECTIVE. After performance of a construction contract begins, Change Orders may be approved if necessary to 1) make changes in plans or specifications; or 2) decrease or increase the quantity of work to be performed or materials, equipment, or supplies to be fumished. Description of change (alteration, deviation, addition, or deletion) caused by conditions encountered during construction not covered by the specifications and drawings of the project (attach additional pages if necessary): This change Order includes the addition of a new electrical duct bank to feed the new BNR electrical building, the removal of the demolition of the washwater detention basin from the contract, the addition of PVC coating to the electrical conduit in the Solids Building, the addition of an end stop on the IMLS monorail, the purchase of a manhole to protect the UV basin chemical drain, the addition of a 6-inch sleeve beneath the road at EPS1, the replacement of mud valve MV-102, the addition of in -situ process monitoring for BNR basin No. 3, the conversion of the hoist trolleys in the Headworks from manual to electric, the addition of a concrete arch encasement on the Plant 3 Filter Influent line, and the realignment of the TWAS pump piping. The total cost of this change order is $247,599,31. The total time extension of this change order is seven days. Change Orders must either be negotiated in the best interest of the City of Lubbock or put out for public bid by the Purchasing Department. Where the Change Order is negotiated, the Change Order must be fully documented and itemized as to costs, including material quantities, material costs, taxes, insurance, employee benefits, other related costs, profit and overhead. The new duct bank RES should have the same conduit schedule as shown in Section 17/E3. Refer to sheet E5. The manhole shall include the same conduits on its north, east, and mm..-. west sides. Paragraph 2-4.02.02 of Section 14622 shall be revised to say: "The trolley drive shall be a sealed worm or spur gear unit, permanently lubricated with an oil bath. Drive wheels shall have integrally cut spur gear teeth or machine -cut gears pressed on hubs. Drive wheel treads shall be hardened. When a festooned power cord type power system is required, the hoist trolley shall have a suitable member to contact and push the bumper of the adjacent conductor cord trolley." Please provide a cost proposal to complete the requested work. The work will not be authorized until a change order has been processed. Box 1600 Keller, TX 76244 Dear Mr. Grant: B&V Project 140092.6100 B&V File G-1.6 December 9, 2009 Subject: Arch Encasement at Plant 3 Filter Influent At the progress meeting on December 2, 2009, we discussed the protection of the joints of the 42-inch Filter Influent line at Plant 3 where is crosses underneath the plant road north of Primary Clarifier No. 1. From our discussion, the pipe supplier is recommending protection of the joints that are located beneath the road. This recommendation comes from the cover of the pipe being approximately 4-feet. Black & Veatch has reviewed the arrangement and we recommend that the pipe beneath the road be protected with a concrete arch encasement. See attached sketches. After reviewing the associated cost, a decision will be made on which Option to incorporate. Option 1: This involves moving the horizontal bends to the discharge piping. The discharge piping will connect to the existing header in the location indicated on MS6. All of the existing pipe as designed would be reused, just in a modified configuration. The 10-112" spool piece (Item 1318 from Ferguson drawing B04217) will be placed on the suction side of the pump, after the two 900 bends. The 8-114" spool piece (Item 1316) will be located between the check valve and the diaphragm valve on the discharge. The modifications apply to all three trains. The Contractor shall submit a cost proposal to make the piping modifications to the train with existing pump P-2502. Option 2: This involves providing a straight run of pipe on both the suction and the discharge side. Most of the pipe would be used in a modified configuration. The 10-1 /2" spool piece (Item 1318) will be placed on the suction side of the pump, after the two 900 bends. The 8-1/4" spool piece (Item 1316) will be located between the check valve and the diaphragm valve on the discharge. The Contractor shall use EBAA Iron series 2100 Megaflanges, or other equal wedge -style restrained flange system, where making the connection of the existing Tee into the new location on the discharge header. A flange system that uses set screws is not acceptable. The modifications apply to all three trains. The Contractor shall submit a cost proposal to make the piping modifications to the train with existing pump P-2502. The cost shall also include the work for cutting into the existing header and reinstalling the existing Tee. A credit should be indicated for not installing Item 1323, 1325, and two pieces of Item 1315. These pieces of pipe would be handed over to the Owner. The Owner will now be responsible for start-up and resetting Pump 2502 switches. Option #2 entails realigning the pumps Suction and Discharge lines to be in line with the Pumps. We have added two existing 6" Diaphragm Valves in the reworked Discharge Header because the Plant Personnel have voiced a wish to have them. The addition of these valves will allow us to keep pumps on line while we perform this work. We will start by installing Pump 2501 1" since the existing pump is about to fail. During the tie-in of this pump all three pumps will have to be off line. Amer this Pump is running and the diaphragm valves are installed then the Plant will have at least one pump on line during the installation of the other pumps.
