WHEREAS, the City Council of the City of Lubbock has determined that it is in the best interest of the citizens of the City of Lubbock to appoint the following named person as the Local Health Authority for the City of Lubbock, Texas, pursuant to Chapter 121 of the Health and Safety Code; NOW THEREFORE: BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK: SECTION 1. THAT the following named person is hereby appointed Local Health Authority for the City of Lubbock, Texas. Donna Bacchi, M.D. SECTION 2. THAT said Local Health Authority shall serve for a term of two (2) years or until such time as a successor has been appointed. SECTION 3. THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute a Certificate of Appointment for a Local Health Authority and any associated documents on behalf of Dr. Bacchi. This form must be completed and submitted to the Texas Department of Health Regional Office together with the Oath of Office form upon appointment of a Local Health Authority. (The validity of the physician's medical license should be verified prior to appointment of the physician as Local Health Authority. As a medical director in the Dallas County Health Department, I overview maternal and child health initiatives and created a mobile - based healthcare for homeless program. In Austin, I was medical director of Community and Neighborhood Services and oversaw numerous public health initiatives- including education efforts to reduce STD's and improve immunization rates. I am currently an Associate Professor of Pediatrics at TTUHSC and the Director of the Center for Tobacco Prevention and Control.
