RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the City Council of the City of Lubbock does hereby approve of the
proposal for the Community in Schools Youth Program from the Community
Development Block Grant (CDBG) program and all related documents. Said proposal is
attached hereto and incorporated in this resolution as if fully set forth herein and shall be
included in the minutes of the City Council.

B. PROPOSED PROJECT:
1. Project Description: In two sentences concisely describe your project. This description will be
used in future publications should your application be approved. Limit two sentences
The purpose of Communities In Schools On the South Plains is to help young people in our
community to stay in school, successfully learn and prepare for life by coordinating resources in
the school setting. A grant for $ 15,750 for the City of Lubbock Community Development
is needed to supportLubbock's only school -based, at -risk youth program in increasing the
number of services received by the youth and by improving the effectiveness of service delivery.
2. List Goals, Objectives and Outcomes. What outcome are you trying to achieve? Please include
the number of people or households that will directly benefit from the program. How will you
measure the outcomes? Please list at least one output and outcome. LIMIT 1/2 page
Project Goals and Objectives of CIS-SP
1) Identify and prioritize personal strengths and needs and translate them into realistic goals.
2) Develop a plan of action for achieving those goals
3) Cross institutional lines, accessing the needed services among a variety of institutions
4) Successfully complete.a customized set of services among a variety of institutions.
5) Help the participant learn to access services on his or her own thereby reducing dependency
on the case manager.
Measurable Outcomes
Outcomes can be measured by students performance in the following:
• Improving school attendance;
• Improving g.p.a.;
• Promoting personal and social development;
• Improving self-esteem;
• Developing work ethic and employment skills;
• Improving behavior;
• Decreasing encounters with juvenile justice system; and
• Increasing parental involvement.
Number of Participants
750 students will participate for case -management and an anticipated 4,500 students total in
Communities In Schools On the South Plains, Inc throughout 2005-2006.

3. List Activity Measures/Steps required to obtain Goals and Objectives. Include a one-year Timeline
from planning to completion for each activity measure. Please highlight significant milestones or
steps of your proposed program. LIMIT 1/2 page
Each summer, CIS-SP generates a strategic nine component Annual Operation
Plan(AOP) for the next academic year and delineates objectives, goals, measures
timelines, and assignments for each category. The following categories that the AOP
addresses are Board Development, Staff Development, Resource Development, Service
Delivery, Volunteers, Administrative, Marketing, Self -Evaluation and Internal
Monitoring, and Expansion. The AOP is reviewed each month to determine and assess
progress and a mid -year report is submitted to the CIS -TX and the Board of Directors.
Fall- Complete Needs Assessment with campus administration
Develop campus plans
Recruit students
Conduct open -house on each campus; teacher in-service on each campus
Winter- Provide case -management and whole school services.
Host Parent's Night to engage parents in their student's lives
Host groups for assistance with TAKS, employment, support, and higher
education
Monitor Grades and Attendance
Mid -year review and surveys and mid -year report
Spring — Continue case -management services
Assist faculty with TAKS preparation
End of year review
4. Evaluation: Explain how you plan to evaluate the program. What criteria will you use to measure if
the program is successful? Please be specific and elaborate on the output and outcome that you listed

The evaluation process of CIS-SP is one of our greatest assets. PEIMS tracking software,
an MS Access based program utilized by Communities In Schools across the state allows
relevant data to be entered to generate tables and queried for reports for specific
information. This system in mandatory for all CIS Texas programs and has proven to be
a successful reporting tool. Student participation, student services, and student outcomes
are entered weekly into the PEIMS, thus providing accurate and ongoing evaluation at
any given time. Student involvement can be tracked, measured and evaluated at any point
during the year. The Youth Coordinator on each CIS-SP campus maintains an ongoing
case file documenting each student's progress. Sign in sheets and attendance rosters are
required for all activities and events. Mid -year and end -of -year surveys are sent to
students, parents, teachers, administrators, and other school staff and faculty. Responses
are entered into a spreadsheet and calculated to identify successful methods and to

improve our program. An Annual Closeout Report provides a complete summary of
students and the services they have received as a result of their work with their Youth
Coordinator.
C. STRATEGY TO DEVELOP ALTERNATIVE FUNDING SOURCES
1. If program is a public service, outline your plan to develop alternative funding to replace Community
Development funds at the end of the three-year cycle. Provide a timeline, with measurable goals and
objectives as a part of your plan. Limit 1 page
2. If this is your second or third year of funding, update your accomplishments in meeting the
goals and objectives in your funding plan and timeline. Limit 1 page
CIS-SP is securing corporate partnerships for programs, and writing grants to many sources
with funding for youth programs including Office of Justice Programs, WorkForce
Investment Act, Texas Council on Alcohol and Drug Abuse, Texas Department of Health and
Human Services to name a few. CIS-SP is continuing to seek support from multiple
foundations.
As CIS-SP services an increasing number of students in the CIS-SP program, additional
funding will be available on the next year TEA contract to cover the cost of services to
additional students. Also, CIS will be made to procure funds from other community
resources, private and public, available for assisting Lubbock students. With the leadership
of the CIS-SP Board and Administrative Office, new areas of funding will be sought to
replace the CDBG funds.
D. PROGRAM INCOME

will create a return of investment to the grantor particularly if proposed project includes

Communities In Schools will be a Public Service program that will not generate income for
the program. It will serve as an effective investment into the lives of Lubbock youth that will
yield great dividends to the community, the targeted neighborhood, participating families,
and the students themselves.
