Station # 13 CITY OF L UBBOCK BI File Number. 17-5005-OM September 19, 2008 GENERAL: The COL Facilities Management Department has requested a proposal for the abatement of VCT flooring and the remodel to Fire Station #13 located at North Fullton and Erskine, Lubbock, TX. 1.0 SCOPE OF WORK: In accordance with conversations with Wesley Everett (COL), Basic Industries, Inc. (BI) and its subcontractors shall furnish all labor, materials, supervision, equipment, insurance, taxes, overhead and all other things or services necessary to complete the above referenced project. 1.1 DEMOLITION & INSTALLATION: The following outlines the requirements for the delivery of this project. Wherein it is stated to "provide" also includes the installation of said components and or systems. 1.1.1A General Demolition: Provide for the demolition of the following to support the above mentioned project per the following outline. Work to include but not limited to the following. There will be asbestos demolition and general demolition for this project. 1.1.113 Demolition: 1. Abatement of approximately 2000 square feet of VCT flooring and mastic. 2. Abatement will be in two phases as coordinated in the preconstruction meeting. 3. Other demolition as needed for the remodel. UAC New Construction: 1. Remove and reset plumbing fixtures. ( Kitchen sink, water heater, gas range, ice machine, 4 lavatory sinks, and 2 commodes) 2. Install new piping for gas stove and relocated ice machine. 3. Relocate 2 X 4 light fixtures to fit new floor plan. 4. Relocate existing recessed down lights to new hallway. S. Install one new wall switch in each of the three new bedrooms. 6. Install one new quad outlet over the desk in each of the new bedrooms. 7. Relocate two existing speakers in the hallway. 8. Construct new divider partition wall as per sight visit and plans by Fire Department, 9. Rework ceiling tile to accommodate new walls. 10. Tape, bed, texture, and paint new walls. 12. Paint all existing walls except in the bay area. 13. Paint all door jambs. 14. Paint shelves, repair cabinets, and restain a needed in sleeping quarters. 15. Stain new doors. 16. Remove and reinstall 7 existing doors. 17. Install new VCT flooring and cove base as selected by owner. Floors will be waxed and buffed. 18. Remove and reinstall cabinets and tops in office, sleeping area, and kitchen area as noted in sight visit. 19. Install new tops in kitchen and restrooms. 20. Repair kitchen counter and shelve where icemaker was located. 21. Install four new desktops. 2.0 SPECIAL CONSIDERATIONS: Special considerations are applicable to this project as outlined in the following: 2.1 The proposal provided herein is based on the assumption that all work can be performed during normal work hours. 2.2 Work shall be performed in accordance with OSHA, Client & BASIC INDUSTRIES requirements. Work areas shall be maintained in a clean and orderly manner with materials, tools, and equipment properly stored and utilized to prevent hazards for worker and incidental personnel in the area. Provide barricades, signs, and other devices as necessary to ensure facility occupants are notified and shielded from dangers that work areas may pose. 3.0 PROPOSAL: Please note that the Proposal does not include any contingency allowance amounts. 4.0 Asbestos: This is an abatement project and all regulations will be followed as per Consultants guidelines. 5.0 TIME OF COMPLETION Basic Industries, Inc. anticipates that the project outlined herein can be completed within (45) calendar days of the Notice to Proceed. 6.0 Time Limit: 6.1 This proposal is good for a period of 30 days from the date of the proposal. Basic Industries reserves the right, prior to issuance of a notice to proceed, to review the cost estimates that forms a part of this proposal and make necessary adjustments as allowed through the City of Lubbock JOC to include quarterly cost adjustments as provided through RS Means. This proposal is based on 2008 RS Means Facilities, Assemblies and Repair and remodeling cost data.
