SUBJECT: Milwaukee Avenue Change Order #6 DATE: June 28, 2006 Council Members: PSC Job No: 01.2872.05 This memo provides a detailed explanation of changes to the Milwaukee Avenue Extension Project Requested in Change Order #6. Item #1— Increase Quantity of Seeding in ROW The original plan quantity did not include seeding within the ROW at cross streets such as 82nd street, 666' Street, Spur 327, US 62/82 and 34`h Street. This additional quantity will cover these areas. Item #1 will add $6,958.50 to the contract. Item #2 — Add Seeding in Channels and Park Area The original plan quantity for seeding did not include seeding the banks of the newly cut channel areas. Because of the large area of seeding added and the difficulty in accessing some of these areas, the contractor has requested a new bid price of $1.88 instead of $1.50 per square yard to accomplish this work. Item #2 will add $71,440 to the contract. Item #3 — Remove Silt from existing 10'x 2' Box Culvert at 82nd Street The adjacent land owner expressed concern in the ability of the existing box culvert beneath 82nd Street west of Milwaukee to carry the intended capacity of water unless the silt is removed from the flowline. The City requested a price from the contractor to remove the silt from the culvert. The cost for completing this work is $6,370 on a lump sum basis. Item #4 — Repair Existing Irrigation Systems The Owner requested that the contractor provide a price for repairing and returning private irrigation systems to pre -construction condition. A total of four systems required repair on properties south of 82nd street on the west side of the road. Granite has retained a contractor to perform these repairs as requested at a cost of $3,570.
