.BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock BE and is hereby authorized and
directed to execute for and on behalf of the City of Lubbock a CONTRACT FOR
TRANSPORTATION SERVICES, attached herewith which shall be spread upon the
minutes of the Council and as spread upon the minutes of this Council shall
constitute and be a part of this Resolution as if fully copied herein in
detail.

COUNTY OF LUBBOCK §
CONTRACT FOR TRANSPORTATION SERVICES
THIS AGREEMENT, entered into as of this 7th day of December, 1978, by and
between the City of Lubbock, hereinafter referred to as the City, and Citizens
for Improved Transportation, Inc., of the City and County of Lubbock, State of
Texas, hereinafter referred to as the "Contractor".
WITNESSETH:
The City and the Contractor do mutually agree as follows:
1. The Contractor shall, in a satisfactory and proper manner as determined
by the City, perform the following services related to the transportation
of persons participating in the Title VII Congregate Meals Program:
A. Provide door-to-door transportation for Agency Centers Monday
through Friday unless advised otherwise one week in advance in
writing.
B. Provide transportation for scheduled field trips for Agency
Centers with two weeks prior written notice.
C. Provide vehicles for special trips after 5 P.M. with one week
notice in writing.
D. A daily record of the names and numbers of people transported
and their destination.
E. A daily record of miles travelled to provide service to the
Agency.
F. The City shall be named as a co-insured on policies for all
title VII vehicles.
2. The City shall furnish the following services, data and information
to the Contractor:
A. Names of persons to be transported to Agency Centers by 5 P.M.
the day prior to placing referred persons or a route in a
mutually agreed upon manner between Contractor and Agency.
3. The Contractor shall commence performance of this Contract on the
1st day of December, 1978, and shall complete performance to the
satisfaction of the Agency no later than the 31st day of May, 1979.
4. The Contractor shall maintain such records and accounts including
property, personnel, and financial records, as are deemed necessary
by the City to assure a proper accounting for all project funds,
both Federal and non -Federal shares. These records will be made
available for audit purposes to the City or the Governor's Committee
on Aging, and will be retained for five years after the expiration
of this contract unless permission to destroy them is granted by
both the City and the Governor's Office.
5. Compensation
A. Monthly Charges
The rate to be paid to the contractor will be figured on a
mileage basis at 33 ents per mile. There will be a monthly
base guarantee of 1, 00 miles.

B. The City will leave the Title to the International bus, (I.D.
#136620H254009) and 12 passenger Dodge Sportsvan, (I.D.
#CGL3670145052) with CFIT, Inc., with the understanding that if
CFIT, Inc. terminates, or does not fulfill the terms of this
contract, the vehicle ownership will return to the City.
6. Method of Payment
The Contractor will bill the agency on a basis of 33 cents per
mile with a minimum monthly base guarantee of 1,400 miles. This
bill is to be received by the City by the 5th of each following
month. The bill is to be filed by the agency on the attached re-
quest for reimbursement form.
7. It is expressly understood and agreed that in no event will the
total amount to be paid by the Agency to the Contractor under this
agreement exceed $5,000 for full and complete satisfactory performance.
8. IN WITNESS WHEREOF, the agency and the contractor have executed this
agreement as of the date first above written.
