78-37-196-515 (43) RESOLUTION BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK: THAT the MAYOR of the CITY OF LUBBOCK, BE and is hereby authorized and directed to execute for and on behalf of the CITY OF LUBBOCK an Application for Project Grant, Grant Agreement and all documents relating to a Grant from the Office on Aging, South Plains Association of Government, attached herewith which shall be spread upon the minutes of the Council and as spread upon the minutes of this Council shall constitute and be a part of this Resolution as if fully copied herein in detail. WITNESSETH THAT: The Agency and the Contractor do mutually agree as follows: 1. The Contractor shall, in a satisfactory and proper manner as determined by the Agency, perform the following: See Attached The Agency shall furnish the following services, data and information to the Contractor: See Attached 2. The Contractor shall commence performance of this Contract on the I day of J&Oce, , 19-771, and shall complete performance to the satisfac- tion of the Agency no later than the day of M.4fj 19-H. 3. It is understood that accountability for equipment purchased with federal funds from this contract remains the responsibility of the appropriate agency administering those funds and any disposition of such equipment must be according to established federal government procedures. The Contractor shall maintain such records and accounts including property, personnel, and financial records, as are deemed necessary by the grantee to assure a proper accounting for all project funds, both Federal and non - Federal shares. These records will be made available for audit purposes to the grantee or the Governor's Committee on Aging, and will be retained for five years after the expiration of this contract unless permission to destroy them is granted by both the grantee agency and the Governor's Office.. These records will be maintained at the following location: Street City r NOTIFICATION OF CONTRACT AWARD & CONTRACT AGREEMENT TITLE VII a) 1) Provide a hot, nutritious meal daily to an average of 240 elderly residents of Lubbock County, with special emphasis to the poverty and minority target groups. 2) Provide supporting services in all centers consistent with Title VII regulations. 3) Provide access by the project director and site managers from each center to training courses on needs and processes of aging as developed and presented by the Office on Aging. 4) The project director will work with the Office on Aging in pre- paring the annual sub -contract. 5) Provide Information and Referral services in all the nutrition centers as well as through the Senior Volunteer Center. Referrals will be sought for the Mayor's Telephone Reassurance Program. 6) Submit required monthly reports and forms with the Request for Payment of Federal Funds. 7) At least one member from the City of Lubbock Title VII project council will be a representative to the South Plains Advisory Council on Aging. 8) On all releases, always credit the Office on Aging, South Plains Association of Governments, as the funding agency. 9) Compliance with Title VI, Civil Rights Act of 1964. 10) Maintain confidentiality of each participant. b) 1) The Project Director, Office on Aging, South Plains Association of Governments, or a designated representative, will be a member of the City of Lubbock's Title VII Project Council. 2) The Office on Aging will be responsible for providing information to the Title VII project director regarding state/federal legislation and policy changes. 3) Will provide technical assistance including program development, staff training, and consultation. 4) Will make monthly monitoring visits and an annual evaluation. 5) Provide demographic data. 6) Provide training related to the aging processes and needs.
