GENERAL INFORMATION This document was developed to provide a uniform format for handling contract changes that affect Contract Price or Contract Time. Changes that have been initiated by a Work Directive Change must be incorporated into a subsequent Change Order if they affect Price or Time. Changes that affect Contract Price or Contract Time should be promptly covered by a Change Order. The practice of accumulating change order items to reduce the administrative burden may lead to unnecessary disputes. For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Time, a Field Order may be used. B. COMPLETING THE CHANGE ORDER FORM Engineer initiates the form, including a description of the changes involved and attachments basal upon documents and proposals submitted by Contractor, or requests from Owner, or both. Once Engineer has completed and signed the form, all copies should be sent to Contractor for approval. After approval by Contractor, all copies should be sent to Owner for approval. Engineer should make distribution of executed copies after approval by Owner. If a change only applies to price or to time, cross out the part of the tabulation that does not apply. Please allow me to compare the requested change orders (my letter of 5/18/98 in regard to excavation quantities and my letter of 5/20/98 in regard to three change order items) with my letter of 2/10/98 (addressed to Victor Kilman). We have to include our costs in the pricing provided to the City. These costs include costs of supervision during installation, cutting and backfilling of anchor trenches, unloading of material when it arrives on site and other administrative expenses. Our pricing at $0.413 / of allows only 3.2Sk to cover these costs. Please give me a call if you have any questions. Thank you. Sincerely, Mike Williams Business Manager 14T ip1 D. E. Box 2000 Lubbock, TX 79457 RE: West Texas Region Disposal Facility - Request for Change Orders. Dear Mr. Hart: As previously discussed, we would like to request the issuance of change orders in three areas. They are as follows: Alternative Bid Item E: The original bid included Alternative Bid Item 9 which provided for an alternative geocomposite drainage layer (with transmissivity of 4.5 x 10-5). This alternate geocomposite drainage layer would be installed instead of the material originally specified and included in bid item #5. Use of the Alternate E material will result in a substantial savings to the City of Lubbock. Following is a•recap of the savings associated with this alternative: Deletion of Bid Item #5 653,395 S.F. @ $0.68 c$444,308.62 Addition of Alt. Due to concerns of this high P.I. material swelling at some point in time and causing problems with the foundations, etc-, the engineer made the decision to overexcavate the areas underneath the building pads and replace the material excavated with a material which was more suitable. In addition, a plastic skirt is to be placed around each foundation to prevent water from corning in contact with the imported material underneath each foundation. Following is our lump such Cost for performing this additional. work: Building Pad Revisions Lump Sum Amount S 9.000.00 4ROM D. E. This being the case, we would like to request that this work be deleted from our contract. Following is the related decrease in contract cost: Deletion of Tree Planting Lump Sum Amount cS IQ. 000.00!> None of these changes will require an extension of contract time.
