HomeMy WebLinkAboutResolution - 2003-R0412 - Affiliation Agreement - Texas Tech University Health Science Center - 10_09_2003Resolution No. 2003-RO412 October 9, 2003 Item No. 23 RESOLUTION BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK: THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute for and on behalf of the City of Lubbock an Amendment and any associated documents to the Affiliation Agreement executed on December 12, 2002, between the City of Lubbock and Texas Tech University Health Sciences Center with regard to Molecular Pathology education, a copy of which Amendment is attached hereto and which shall be spread upon the minutes of this Council and as spread upon the minutes of this Council shall constitute and be a part hereof as if fully copied herein in detail. MAAC MUbqGAL, MAYOR ALAR 03066 A Resolution No. 2003-RO412 October 9, 2003 AMENDMENT Item No. 23 This amendment is made to that agreement ALAR 03066-0 previously executed by and between Lubbock City Health Department, hereinafter called "Facility", and TEXAS TECH UNIVERSITY HEALTH SCIENCES CENTER, School of Allied Health, Department of Laboratory Sciences and Primary Care, hereinafter called "TTUHSC". It is mutually understood and agreed by and between the undersigned contracting parties to amend that previously executed agreement as follows: Article II, Paragraph a. add the following: ix. Provide orientation for TTUHSC's students to inform them of the Facility's policies (i.e. safety), procedures, rules, and regulations. x. Arrange for emergency health care for a student if needed, at student's own expense, while the student is on -site at Facility, provided however, that Facility shall not be responsible for costs, follow-up care, or hospitalization associated with such emergency care. Article II, Paragraph b. add the following: vii. Make available the Student Health Service of TTUHSC for all students enrolled in the program. The Facility does not assume responsibility for provision of health services, medical care or treatment of injury for students. Article II, add the following: c. Joint responsibilities i. TTUHSC and Facility liaisons will meet as needed to review the progress of the student clinical experience and mutually develop improvements in the program. ii. It is agreed that the parties will not discriminate in their employment, training, or education of any persons or in the condition of services or in other actions pursuant to this agreement by reasons of age, race, religion, or natural origin, nor by reason of any form of disability. All other terms and conditions that are not hereby amended are to remain in full force and effect.
