That the City of Lubbock has received a proposal addressed to its Mayor and City Council for professional engineering services in the form of engineering laboratory testing services to be done by Terra Testing Inc. in connection with the 34th Street and Quirt Avenue Overpass Con- struction Project, to be accomplished for an approximate estimated total cost of $18,740.00 adjusted to the actual cost for the work in line with the rates for services as set forth in said proposal, a copy of which proposal is attached hereto and made a part hereof as Exhibit A. Section 2. The City Engineer, at his sole discretion, may allow the Resident Project Representative to notify and request engineering labora- tory testing services from Terra Testing Inc. as the need of such service is required to serve the best interests of the City of Lubbock. Boa 2000 Lubbock, Texas 79457 Re: City of Lubbock Quirt Avenue Overpass Dear Mr. Bertram: We are pleased to advise you that the resident project representative we are planning to assign to the referenced project is Mr. Robert J. Field. Mr. Field has been 'involved in the heavy construction industry for more than thirty years. Mr. Field has served as resident project represen- tative, survey party chief, estimator and construction superintendent on numerous construction projects involving the construction of bridges, high- ways, streets, drains, sewers, reservoirs, water and wastewater improve- ments, airports and dams. Mr. Field's experience includes work in Massachusetts, Maine, New York, Illinois and Texas. Mr. Field was employed by Parkhill, Smith & Cooper, Inc. in April, 1982 and has served as resi- dent project representative on various water, wastewater, street and air- port construction projects in Lubbock and the surrounding area. Mr. Field's experience in construction of highway bridges and other heavy construction projects qualifies him to serve in the capacity of resident project representative on the Quirt Avenue Overpass, coupled with periodic site visits by a representative of the bridge design division of Henningson, Durham & Richardson. We also wish to advise you that we plan to use Wilson Surveying Co. for construction surveying needs. The construction contract requires the Contractor to furnish all surveys necessary to provide horizontal and ver- tical lines and grades to construct the work. Any construction period sur- veys performed by Wilson Surveying Co. for the City will be authorized through our office, only for "spot-checking" the accuracy of staking and construction performed by the Contractor. The testing laboratory we plan to use is Terra Testing, Inc. The lab will only test materials delivered to the site and report results of material tests required to insure compliance with the project specifications. Box 2000 Lubbock, Texas 79457 Attention: Mr. Jim Bertram, Assistant City Manager Gentlemen and Mrs. Baker: In reference to the 34th Street and Quirt Avenue Over- pass Project, Terra Testing is pleased to submit this proposal for construction materials testing services. The quantities listed below are estimates of the number of tests required as provided by HRR.,-; Engineers. Terra Testing has not made a separate estimation of quantities or types of tests required. There is no charge for mileage within the City of Lubbock. The charges below, then, include technician's time, laboratory test- ing, transportation of samples, calculations and reports, copies and postage. The forty (40) hours extra itemization for technician's time is intended to cover the charges for technician waiting time beyond the one hour allowed per trip. These unit prices shall remain good throughout the duration of the project, up to fourteen (14) months from the date construction begins. It is our polity to provide those services required and requested by the City Engineer or the City Engineer's designated representative. We would further request that the City of Lubbock, as our client, provide us with a list of those firms approved to re- ceive copies of our reports. Terra Testing Inc. is insured through Fred S. James & Company of Texas, Inc., underwritten by Lloyds of London. This policy is our professional liability insurance coverage in the amount of $500,000.00. A certificate of coverage will be provided at your re- quest. Of course, Terra Testing also carries the other customary business insurances. Proof of coverage is also available.
