HomeMy WebLinkAboutResolution - 4769 - Pre-Application - TSL, Title II - Grant Funding, Northwest Branch Library - 03_09_1995Resolution No. 4769 March 9, 1995 Item #14 BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK: THAT the Mayor of the City of Lubbock BE and is hereby authorized and directed to execute for and on behalf of the City of Lubbock a Pre -Application to Texas State Library for grant funding under Title H, Library Services and Construction Act, for the Northwest Branch Library and Family Learning Center, attached herewith, by and between the City of Lubbock and Lubbock City -County Library, and any associated documents,. which Application shall be spread upon the minutes of the Council and as spread upon the minutes of this Council shall constitute and be a part of this Resolution as if fully copied herein in detail. May Preliminary recommendations for funding mailed to all applicants. Applicant's response, if any, due at the Texas State Library, June Successful applicants submit Title II application. Texas State Library forwards recommended projects to Washington. July Texas State Library and Archives Commission approves all grants. August Contracts between the Texas State Library and grant recipients issued following approval by the U.S. Department of Education. September 1 Projects begin. PREAPPLICATION CHECKLIST While it is important to provide all the information requested on the Preapplication and the Guidelines, failure to provide the minimal information listed below could result in rejection of the Preapplication. Narrative Should Include -- gross square footage of existing public library building(s) owned by city or county -- square footage of the proposed library building -- all in -kind and actual cost estimates clearly differentiated -- project timetable -- name of state senator and state representative Project Cost Estimates Cost estimates should be realistic to ensure that the library building can be completely constructed, fumished, and equipped for opening day. Preapplication Submittal TWO (2) signed copies of the Preapplication submitted to the Texas State Library. The Preapplication should be signed by an official authorized to enter into agreements binding on the city or county. Please note that the Preapplication references the assurances found in the Guidelines, Part XI. Additional Steps to Take The Guidelines, Part VIII.A., lists additional steps that you must take at this time. Other Costs (Site preparation, Soil tests, etc.) -0- 5,000 5,000 TOTALS o -0- a 1,457,502 o 1,457,502 TEXAS STATE LIBRARY, LSCA TITLE II PREAPPLICATION Page 3 PART IV - PROGRAM NARRATIVE The brief narrative of the proposed project should include the following information: ♦ Type of project (new, conversion, expansion, disability access remodeling, etc.). ♦ Gross square footage of existing facility, and gross square footage of proposed facility (if the project involves a branch or remodeling for disability access, give the gross square footage of all owned facilities). ♦ If the project is an expansion, give square footage to be added. ♦ A statement of who owns the existing facility, and who owns the land on which the proposed facility will be built or converted. ♦ List by source (cash, bonds approved by voters, foundation grants, CDSG funds, etc.) the total amount of local project costs on hand at the time of pre -application. Exclude in -kind contributions. ♦ A brief project timetable. ♦ The names of your state senator and state representative. 1) Central Library, 72,000 sq. ft. The applicant will be the name of your library, and the contact person should be the individual with whom the State Library will work throughout the project. Block . 2. Provide a brief description of the proposed construction project. Block 3. List the amount of proposed funding by source. The total proposed funding should equal the total given in Part 111, No. 6. Block 4. The Federal Employer Identification Number is the number assigned by the Internal Revenue Service. Block 6. Give the name of your regional council or planning commission. A copy of the preapplication should be submitted to your regional council or planning commission requesting its review. An identifier number will be assigned by the council or planning commission. Block 7. The population served is the population service area assigned by the State Library under the Rules for Administering the Library Systems Act. Block 8. A response must be given to both questions. Block 9. Leave blank. Block 10. Signature of the city manager, mayor, or county judge who is empowered to enter into agreements binding upon the city or county government. PART 11 Questions 1, 2, 4, 5, and 6 in all probability will not pertain to your preapplication. Question 3. A review is required by the regional council of government or planning commission. See Part 1, Block 6 above. Questions 7 through 9 should be answered. Any affirmative responses must be explained in the narrative, Part IV. PART III - PROJECT BUDGET Block 1. If in -kind contributions are included in the project, the amount should be entered in the appropriate category, i.e., land acquisition, furniture, etc. Any in -kind contributions must also be fully explained in Part IV, Program Narrative. Block 2. List the amount of actual cost estimates for each of the items 1 through 5. Block 3. List the total amount of in -kind and actual cost for each of the items 1 through 5. Block 4. List the total of all in -kind contributions. Block 5. List the total of all actual cost estimates. Block 6. List the total of all in -kind contributions and actual cost. This total should be the same as the amount entered in Part 1, Proposed Funding, Block 3g. PART IV - PROGRAM NARRATIVE In addition to the specific information requested on the Program Narrative form, a listing of all in -kind contributions should be included. Please make the narrative as concise as possible, providing only the pertinent information requested. Do not include supporting illustrations, brochures, etc.
