RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute for
and on behalf of the City of Lubbock, Amendment No.3 to the Office Lease for additional
square footage for City of Lubbock Public Health Department Services, by and between
the City of Lubbock and Housing Authority of the City of Lubbock, Texas, and related
documents. Said Amendment to the Office Lease is attached hereto and incorporated in
this resolution as if fully set forth herein and shall be included in the minutes of the City
Council.

AMENDMENT NO.3 TO OFFICE LEASE
WHEREAS, on June 21, 2012, the City of Lubbock (as Tenant) entered into an Office Lease
with the Housing Authority of the City of Lubbock (as Landlord);
WHEREAS, the Office Lease set forth the Premises to be leased to the Tenant as Approximately
5800 square feet in the Lubbock Housing Authority Administrative Building located at 1708
Crickets in Lubbock, Texas 7 940 1;
WHEREAS, the Office Lease set forth the Base Rent (monthly) as $3,383.34 based on 5800
square feet at$7.00 per square foot;
WHEREAS, the Office Lease provided that the lease may be amended only by an instrument in
writing signed by Landlord and Tenant;
WHEREAS, the Tenant and Landlord previously amend the Office Lease to include an
additional 528 square feet for three (3) additional offices next door to the office space occupied
by the Health Department at an additional monthly rent of$352 calculated at $7.00 per square
foot plus $1.00 per square foot for utilities, making the total monthly rent $3,735.34,-
WHEREAS,
3,735.34;WHEREAS, the Tenant and Landlord amended the Office Lease a second time to include an
additional 162 square feet of space occupied by the Health Department at an additional monthly
rent of$108 calculated at $7.00 per square foot plus `61.00 per square foot for utilities, making
the total monthly rent $3,843.34; and
WHEREAS, the Tenant and Landlord wish to amend the Office Lease to include an additional
522 square feet of space occupied by the Health Department at an additional monthly rent of
$348 calculated at $7.00 per square foot plus $1.00 per square foot for utilities, making the total
monthly rent$4,175.34, leaving all other terms of the Office Lease as they are;
NOW THEREFORE, the following terms of the above-described Office Lease are hereby
amended as follows:
1) Premises
Approximate square feet: 7012 square feet
Name of building: Lubbock Housing Authority Administrative Building
Street address/suite: 1708 Crickets
City, state, zip: Lubbock, Texas 79401
Base Rent (monthly): $4,175.34 (based on the original 5800 square feet at $7.00
per square foot and $7.00 per square foot plus $1.00 per

square foot for the utilities for the additional 528, 162 and
522 square feet.)
There shall be no additional Security Deposit and no additional charge for utilities.
The additional rent shall be due beginning in the first month for which the Tenant takes
possession of the additional space.
2) That Section E. 20. of the Office Lease is hereby substituted and replaced by the
following language:
20. Tenant's Option to Reneiv, Termination.
a. Tenant is granted and will, if not then in default under this lease, have an
option to renew this lease for two additional five year terms from the lease
expiration date and the first renewal expiration date, but the rent will be at
such monthly rental as may be negotiated between Landlord and Tenant
prior to that time. This option may be exercised only by Tenant delivering
to Landlord, in person or by mail ninety (90) days before the lease or first
renewal term expires, written notice of the election to renew this lease as
provided.
b. Landlord or Tenant may, at its election, terminate this lease by giving the
other party thirty (30) days written notice that lease is terininated. Such
written notice must state the reason for cancellation
