RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock is hereby authorized and directed to execute
for and on behalf of the City of Lubbock a Memorandum of Agreement ("MOA") by
and between the City of Lubbock and Managed Care Center for Addictive/Other
Disorders, Inc. regarding the HIV Early Intervention Program. Said MOA is attached
hereto and incorporated in this Resolution as if fully set forth herein and shall be
included in the minutes of the Council.
Passed by the City Council on December 7 2017.

MEMORANDUM OF AGREEMENT
Managed Care Center for Addictive/Other Disorders, Inc. — HIV Early Intervention
Program (HEI) and City of Lubbock Health Department are both committed to assisting
members of our community in a cooperative effort by helping individuals who are HIV positive and
have a history of or current problem with substance abuse. We recognize that by working together
we can encourage and support these individuals to seek early detection with treatment focus.
Promoting the engagement in activities that will focus their attention and energy on the positive
aspects of life. To accomplish this goal we agree to the following:
Managed Care Center, Inc. HIV Early Intervention Program will:
♦ Accept referrals for individuals who are HIV positive and report having a history of or current
substance abuse problems.
♦ Provide case management to address client needs including medical, substance use, and social
services.
♦ Provide access to substance abuse treatment as indicated.
♦ After obtaining proper consent in accordance with Federal Register 42CFR and HIPPA, HEI
Staff will share required information needed for referral and follow-up.
♦ Coordinate services with the City of Lubbock Health Department -Street Outreach Team and
other appropriate community resources.
City of Lubbock Health Department will:
♦ Accept referrals from Managed Care Center for HEI clients needing Immunizations services or
provide appropriate referrals when necessary.
♦ Refer individuals who are HIV positive and have a history of substance abuse or current
substance abuse problems to the HEI program for services.
♦ Coordinate services with HEI case manager as needed.
♦ Provide assistance with health focused issues as needed by the citizens of Lubbock also provide.
education on their programs such as Immunizations,Refugee, Community Disease,Health
advisories and emergency preparedness (PREP) as needed.
This agreement is effective September 1, 2017 and expires August 31, 2018.
