RESOLUTION
WHEREAS,the City Council of the City of Lubbock has determined that it is in the best
interest of the citizens of the City of Lubbock to enter into an agreement with StarCare
Specialty Health System to refer identified individuals who meet the priority or target
population to StarCare for education,outreach,family services,referrals and/or treatment.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor is hereby authorized and directed to execute the StarCare Specialty Health
System Memorandum of Understanding and any associated documents.
Passed by the City Council on this 14th day of September ,2017.

Memorandum of Understanding
THIS MEMORANDUM OF UNDERSTANDING is entered into by and between the agencies identified below.
I. AGENCIES:
The Performing Agency: Lubbock Regional MHMR Center
DBA StarCare Specialty Health System (StarCare)

The Receiving Agency: City of Lubbock Health Department

II. STATEMENT OF SERVICES TO BE PROVIDED:
Through collaboration with StarCare, City of Lubbock Health Department will identify and refer individuals meeting
the definition of priority and target population to StarCare. Through collaboration with the City of Lubbock Health
Department, StarCare will provide education, outreach, family services, referrals, and/or treatment to identified
individuals meeting priority or target population criteria
III. THE PERFORMING AGENCY AGREES TO:
(a) Collaborate with Receiving Agency to identify individuals who meet the Department of State Health
Services (DSHS) criteria of priority or target population (refer to section V. for definitions);
(b) Provide staff, time and resources that will empower the community in developing a natural support system;
(c) Share available education and training opportunities;
(d) Provide access to productive, supportive services and activities offered through the StarCare network when
appropriate (such as intervention, treatment, education, outreach, social/recreational activities, family
services and access to HIV/AIDS and drug testing);
(e) Provide healthy adult role models who promote pro -social values, attitudes, and behavior;
(f) Collaborate with Receiving Agency to identify duplication of services and/or gaps in services within the
community; and
(g) Collaborate with Receiving Agency to develop strategies to close gaps in services, prevent duplication of
services and to ultimately meet the needs of the priority and target population.
IV. THE RECEIVING AGENCY AGREES TO:
(a) Help identify individuals who meet the priority or target population criteria and link them to Performing
Agency's services;
(b) Collaborate with the Performing Agency to empower the community in developing a natural support system;
(c) Collaborate with the Performing Agency to identify duplication of service and/or gaps in services within the
community; and
(d) Collaborate with Performing Agency to develop strategies to close gaps and prevent duplication of services
and to meet the needs of the priority and target population.

V. PRIORITY AND TARGET POPULATION DEFINITIONS:
(a) Adult Mental Health Priority Population: Adults who have severe and persistent mental illnesses such as
schizophrenia, major depression, bipolar disorder, or other severely disabling mental disorders which require
crisis resolution or ongoing and long-term support and treatment.
(b) Adult Mental Health Target Population: Adults who have a diagnosis of schizophrenia, bipolar disorder, or
severe major depression.
(c) Child and Adolescent Mental Health Priority Population: Children ages 3 through 17 with a diagnosis of
mental illness who exhibit serious emotional, behavioral or mental disorders; and who (1) have a serious
functional impairment; (2) are at risk of disrupting a preferred living or child care environment due to
psychiatric symptoms; or (3) are enrolled in a school system's special education program because of a
serious emotional disturbance.
VI. TERM OF AGREEMENT:
This Agreement is effective as of September 1, 2017 and shall terminate August 31, 2018.
Both the Performing Agency and the Receiving Agency mutually agree that either party may terminate this
Memorandum of Understanding with a thirty (30) day written notice.
THE UNDERSIGNED AGENCIES do hereby certify that the person or persons signing and executing this Memorandum of
Understanding warrant and guarantee that they have been duly authorized by their respective Agency to execute this
Memorandum of Understanding and to validly and legally bind the Agency to all the terms and provisions of this
Memorandum of Understanding.
