Said Contract is attached hereto and incorporated in this resolution as if fully set forth herein and shall be included in the minutes of the City Council. POPE, MAYOR Resolution No. 2021-R0267 AMENDMENT 1 TO AGREEMENT BETWEEN THE CITY OF LUBBOCK FRO CEAP HEATING AND COOLING UNIT SERVICES THIS IS THE FIRST AMENDMENT TO THE AGREEMENT, Contract No. 15364 dated and entered into the 1 l" day of August 2020 by and between the City of Lubbock ("City") and Off Plants A/C Heating (Contractor) of Lubbock, Texas. The Contractor entered into the Agreement with the City to provide the CEAP Heating and Cooling Unit Services per ITB 20-1515364-KM. Amendment No. 1 increases the original Agreement amount of $54,873.00 by an additional $4,322.00 with the expiration date of the Agreement remaining as originally stated. The new contract amount will be $59,195.00 in accordance the revised bid form attached as Exhibit A and incorporated herein. All other portions of the original Agreement terms and conditions shall remain in place and are not altered by this amendment. I ELECTRICAL REPAIRS Bid ITB 20-15364-KM Amendments - Electrical Repairs Quantity Units UNIT COSTS EXTENDED COST LICENSED CONTRACTOR AND/OR LICENSED SUBCONTRACTOR IS RESPONSIBLE FOR OBTAINING ALL REQUIRED CITY OF LUBBOCK BUILDING CODE PERMITS AND FEES PAID, AS NEEDED TO PROPERLY COMPLETE THIS PROJECT. THE GENERAL CONTRACTOR MUST NOTIFY COMMUNITY DEVELOPMENT HOUSING INSPECTOR OF EACH ISSUED PERMIT AND PERMIT FINAL APPROVAL. CONTRACTOR IS RESPONSIBLE FOR ALL ASSOCIATED COMPONENTS FOR A PROPER AND COMPLETE INSTALLATION. CONTRACTOR SHALL APPLY ALL NECESSARY CODE UPDATES. (Example; number of and/or spacing of receptacles) All materials shall be new unless otherwise specified, and UL approved and/or National Electrical Code rated. All drilling, cutting and fastening shall be neat and true. All patching shall match the existing or surrounding surface. All receptacles and cover plates shall match color and style, as required. a.1 MAIN ELECTRICAL SERVICE with PANEL: REPLACE Install 125 - amp single phase, 3-wire electric service. Include a 20 circuit exterior panel surface mounted in place of the existing interior panel. Include a blank panel cover for the existing interior panel. If existing panel cover is missing, new blank cover shall be a minimum of 1/2" thick MDF material, squarely cut to appropriate size to fit in place of the panel cover and securely installed to interior panel cover mounts to prevent access. If the existing panel cover is available, new blank cover shall be a minimum of 22 gauge steel, squarely cut to appropriate size to fit in place of the punchout plate and securely installed in place of the punchout plate to prevent access. Existing panel cover to be re -installed over new blank cover. Add circuits in the panel as required to accommodate additional newly installed wiring and receptacles. Include a main disconnect (properly sized copper wire and breakers). Contractor shall install adhesive backed labels filled out for each breaker. Include a new meter base in same location of existing. Install a grounding rod and new mast to ensure incoming electrical is at least 10' off of grade. If there is any aluminum wiring, wiring must have anti -oxidant compound on the terminal connections. Disconnect the incoming main service for ROOFER, flashing installation over mast and then reconnect main incoming electrical service (10' minimum height from the finished grade). Electrical service weatherhead is to be over roof with proper heights and have the proper flashing and mounting. Surface mount new panel on exterior wall near incoming service. 1 All a.4 MIAN DISCONNECT Install a new main disconnect (properly sized copper wire and breakers). O 1 All a.5 A/C CONDENSER DISCONNECT Add a new A/C disconnect and ensure wiring at the panel is proper for the breaker size for the outside A/C condensing unit. FOLLOW THE MAXIMUM SIZE AS RECOMMENDED BY THE MANUFACTURER. 1 All a.6 WALL HEATER CIRCUIT Run new wiring (properly connected and breakered) to the wall heater area. Connect the electrical to the newly installed wall heater. NOTE: DO NOT INSTALL 3-PRONG ON AN UNGROUNDED 2-WIRE SYSTEM. 1 All a.7 REFRIGERATED WINDOW UNIT RECEPTACLE Run new wiring (properly connected and breakered) to the location of window A/C unit -window. Install a new grounded receptacle at window. NOTE: DO NOT INSTALL 3-PRONG ON AN UNGROUNDED 2-WIRE SYSTEM. 1 All a.8 GROUNDING ROD and GROUND BUSS Install a new ground buss inside the existing electrical panel. Include grounding new buss and panel to new grounding rod. NOTE: DO NOT INSTALL 3-PRONG ON AN UNGROUNDED 2-WIRE SYSTEM. 1 All - _ - UNIT Item ITEM Quantity Units COSTS EXTENDED No. Mat./Lab. COST a.9 RECEPTACLES: TROUBLESHOOT Ensure receptacles near furnace are properly functioning. In mechanical closet or mounted near horizontal furnace in attic, replace receptacles for outlets as needed. All receptacles and cover plates shall match color and style- Replace with proper outlet, if original is a 2-prong system or a 3- prong system with proper grounding.
