RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock BE and is hereby authorized and
directed to execute for and on behalf of the City of Lubbock a Notification
of Contract Award and Contract Agreement related to the Grant Application
for VII Nutrition Program, attached herewith which shall be spread upon the
minutes of the Council and as spread upon the minutes of this Council shall
constitute and be a part of this -Resolution as if fully copied herein in
detail.
Passed by the City Council this 28th day of June ,1979.

THIS AGREEMENT, entered into as of this 1st day of dune 19 79
by and between Office on Aging, South Plains Association of Governmentef the City/
County of Lubbock , State of Texas hereinafter
referred to as the "Agency", and City of Lubbock of the City/
County of Lubbock , State of Texas ,
hereinafter referred to as the "Contractor". WITNESSETH THAT:
The Agency and the Contractor do mutually agree as"follows:
1. The Contractor shall, in a satisfactory and proper manner as determined
by the Agency, perform the following:
la. See written agreement consolidating the City of Lubbock's Title VII
Nutrition Project with the South Plains Office on Aging (An Area
Agency on Aging)
The Agency shall furnish the following services, data and information to
the Contractor:

2. The Contractor shall commence performance of this Contract on the st
day of June , 1979 , and shall complete performance to the satisfac-
tion of the �Agency no later than the 31st day of May 19EL.
3. It is understood that accountability for equipment purchased with federal
funds from this contract remains the responsibility of the appropriate
agency administering those funds and any disposition of such equipment must
be according to established federal government procedures.
The Contractor shall maintain such records and accounts including property,
Personnel, and financial records, as are deemed necessary by the grantee
to assure a proper accounting for all project funds, both Federal and non -
Federal shares. These records will be made available for audit purposes
to the grantee or the Governor's Committee on Aging, and will be retained
for five years after the expiration of this contract unless permission to
destroy them is granted by both the grantee agency and the Governor's
Office. These records will be maintained at the following location: e

AGREEMENT, REVISED JUNE 12, 1979,
CONSOLIDATING THE CITY OF LUBBOCK'S
TITLE VII NUTRITION PROJECT WITH
THE OFFICE ON AGING AN AREA AGENCY
ON AGING), SOUTH PLAINS ASSOCIATION
OF GOVERNMENTS.
1. Consolidation of the City of Lubbock's Title VII Nutrition Project
(CLTVIINP) will occur on June 1, 1979, the date for refunding of the
project.
2. The South Plains Office on Aging (SPOA) will submit the A-1; A-2; A -2a;
D-2; and D-3 forms; and Exhibit D-6 in the area plan to complete the
consolidation process.
3. The Governor's Committee on Aging (GCOA) will list the SPOA as the PAYEE
on the Notification of Grant Award from the GCOA.
4. The SPOA will provide nutrition and support services to the CLTVIINP as
requested and required.
5. All fiscal and programmatic reports will originate with the CLTVIINP and
will be submitted through the SPOA who will be responsible for their
timely and correct completion.
6. Programmatic Reports, for the preceeding month, including the Commodity
Report, and the Advance Request for Federal Funds (T-201), based on projected
needs for the current month, shall be submitted to the Area Agency on Aging
by the CLTVIINP on or before the third (3rd) of the month. The Statement
of Monthly Costs, with copies of invoices, shall be submitted to the Area
Agency on Aging by the 15th of the month following the period for which the
costs were incurred.
The final expenditure report (T-201) shall be submitted no later than
thirty days following the close of the contract year.
7. The Area Agency on Aging shall prepare the State of Texas Purchase Voucher,
and supported by the T-201 Request for Advance of Federal Funds, remit same
to the GCOA.
8. On receipt of the Advance Payment of Federal Funds from the GCOA, the Accounting
Department, SPAG, will authorize payment to the CLTVIINP on the next scheduled
accounts payable date.
9. The SPAG Accounting Department will mail the check to the CLTVIINP on
issuance by the computer processor.
