HomeMy WebLinkAboutResolution - 283 - Application/Contract Award & Agreement-SPAG-Title III Residential Repair Project - 09/27/1979RESOLUTION #283 - 9/27/79 DGV : c 1 RESOLUTION BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK: THAT the Mayor of the City of Lubbock BE and is hereby authorized and directed to make application for a Title III Residential Repair Project Grant and to execute for and on behalf of the City of Lubbock a Notification of Contract Award and Contract Agreement-- Title III, between the South Plains Office on Aging of Lubbock, Lubbock County, Texas, and the City of Lubbock, Texas, attached herewith which shall be spread upon the minutes of the Council and as spread upon the minutes of this Council shall constitute and be a part of this Resolution as if fully copied herein in detail. Box 2000 Inhhork- TPxac 7Q457 NOTIFICATION OF CONTRACT AWARD & CONTRACT AGREEMENT TITLE III i THIS AGREEMENT, entered into as of this 1st day of July 19 79 by and between the Office on Aging, South Plains Association of Governments, Lubbock, Lubbock County, Texas, hereinafter referred to as the "Agency" and Community Services Department - City of Lubbock hereinafter referred to as the "Contractor". WITNESSETH THAT: The Agency and Contractor do mutually agree as follows: 1. The Contractor shall, in a satisfactory and proper manner perform the following: See page 2, continuation # 1 2. The Agency shall furnish the following services to.the Contractor: See page 2, continuation # 2 3. The Contractor shall commence performance of this Contract on the 1st day of July , 19 79 , and shall complete performance to the satisfac- tion of the Agency no later than the 29th day of February 19 4. It is understood that accountability for equipment purchased with federal funds from this contract remains the responsibility of the appropriate agency administering those funds and any disposition of such equipment must be according to established federal government procedures. 5. The Contractor shall maintain such records and accounts including property, Personnel, and financial records, as deemed necessary by the Agency to assure a proper accounting for all project funds, both Federal and Non - Federal shares. These records will be made available for audit purposes to the Agency or the Governor's Committee on Aging, and will be retained for five years after the expiration of this contract unless permission to destroy them is granted by both the Agency and the Governor's Office. These records will be maintained at: Page 2 - Continuation, #'s 1 and 2 CONTINUATION, # 1: 1. Provide residential Repair services for a minimum of 12 homes for citizens over 60 years of age, as described in contract application. 2. Attend all training events, and participate in region wide events sponsored by Area Office on Aging. 3. Maintain and submit necessary forms and reports. 4. Maintain confidentiality of each participant. 5. Comply with Title VI, Civil Rights Act of 1964. 6. Coordinate agency's Outreach services to minority and low income groups to include information about residential repair services. f 7. Provide matching funds as described in Title III contract application. w r 8. When releasing information about the Contract, identify the Office on Aging, South Plains Association of Governments,as the funding agency. CONTINUATION, # 2: 1. Furnish Title III funds as described in contract application. 2. Furnish technical assistance, through site visits and telephone contacts, as needed. 3- Monitor and evaluate the program. 4. If any personal property is so provided, this assurance shall obligate the Subgrantee for the period during which it retains ownership or possession of the property. In all other cases, this assurance shall obligate the Subgrantee for the period during which the Federal .Financial assistance is extended to it by the Grantor.
