RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK:
THAT the Mayor of the City of Lubbock BE and is hereby authorized and
directed to execute for and on behalf of the City of Lubbock a Contract to
provide preventive health care services between the Texas Department of Health
and the City, attached herewith which shall be spread upon the minutes of the
Council and as spread upon the minutes of this Council shall constitute and be a
part of this Resolution as if fully copied herein in detail.
Passed by the City Council this 27t h day of September ,1979.

Health Department of
Lubbock regarding her.wish to become a Title XIX E.P.S.D.T. provider.
Dr..Orr is interested in the following:

5. Neonatal Assessment
A. Examination of child in clinic at two weeks. i
B. Nurses - make home visits
Dr. Orr has proposed several changes for the future to insure higher quality
of service"to the clients. She has requested funding to hire three social
workers and is erecting to receive a grant in October of 1979 to begin
a maternal and Child Health Program in Lubbock county. Dr. Orr was involved
in the E.P.S.D.T.program in Virginia and is most anxious to become apart
of Texas E.P.S.D.T.

The Texas Department of licalth, hereinafter called the "Receiving Party", acting
by and through its Deputy :onmrissioner of the Department (if llealth, and the City of
Lubbock, a municipal cor\\ r tions , Texas, hereinafter
called the "Performing Pa -y , acting by and thrOUgh its Department of Health,
has jointly considered and a rmined that a contract should be entered into under
the provisions of which the �erformLL%_ Party will provide preventive health services
within Lubbock City and County, to 1'a income materntty patients, newborn infants,
and rrc-school children, tnclu� ng, tut not restricted to, 11.i.G. recipients.

A. Provide preventive health care servv`lw _g. sictan, nursing, social,
and nutritional assessment and refer 1, medically indigent maternity

County primarily through coopera on ith Texas Tech university School of
.Medicine, Lubbock County lios al Dist ict, and the Lubbock United Way

be carried out in accords a wth the les, regulations, policies and
guidelines as promulgated by. Fe era and State contracting agency
and in accordance with the Improv Chi d Health Project Guidelines.
B. Abide by the following policy on "Grant 1 ated Income" and "Third Party
Reimbursements":
1. Charges to any other funding sources miis-t"68-itandled by one of the
following methods:
a. Except as othe ise pro ided Herein, no item which has been charged
to any other f nding so rce c is rg •(l -c-o the Receiving Party
under this con race.
b. Receipts from an)) whic s also been charged to the receiving
Party under this Contract must he idered as grant -related in-
come.
(1) Grant-relat . income must be -counted for in the budget in
whish it i earned, and such in we must be reported to the
Receivin Party semi-annually.
(2) . Unspen grant -related income at,the c se of a budget period
may b carried into the next budget period with approval of
the R ceiving Party.
(3) All rant -related income. carried, from pti r budget periods

2. If there is more than a single funding source:, all rant -related in-
come must be apportioned among funding sources and reported to the
Receiving Party semi-annually.
3. Any income derived through the use of paid positions of'the Receiving
Party must he forwarded to the Receiving Party.
