Answer every point; if an item does not apply to your project, explain that it does not apply. 1. State that, as a grantee, you will maintain compliance with other applicable Federal, statutory and regulatory requirements as set forth in 40 CFR Part 30, Subpart C. .3. _ 3 CG -122C TEXAS ( Rev. 3-7-80) Check When Application Complete EPA Form 5700-32 2. Give the name, address and telephone number of your prime consultant. 3. Explain how you intend to award any subcontracts. 4. Is this project located in an identified Flood Hazard Area? If so, have you taken necessary steps to secure DHUD Flood Insurance coverage before applying for Step 3 grant assistance? 5. Will a Federal facility discharge to this wastewater treatment plant? (Refer to 40 CFR 35.925-16) 6. Does your waste discharge permit require treatment more stringent than secondary? 7. a. Is a pretreatment program required to be developed for this facility under 40 CFR 403.8(a)? b. If a pretreatment program must be developed then the items required by 40 CFR 35.907(d)(1)-(9) must be submitted at this time, for grantees who will receive Step 3 grants after December 31, 1980. 8. Grant Payments State that you will request grant payments monthly. Remember, however, a request for payment should be no greater than the Federal share of costs incurred at the time the request is submitted. All applicable federal, statuatory and regulatory requirements as set forth in 40 CFR Part 30, Subpart C will be complied with. 2. The name, address and telephone number of the prime consultant is listed below. PARKHILL, SMITH & COOPER, INC. 4010 Avenue R Lubbock, Texas 79412 Phone: (806) 747-0161 3. The City of Lubbock intends to utilize the consulting engineer- ing firm of Parkhill, Smith & Cooper, Inc., as prime consultant for the Step 3 phase of the project. This firm provided Step 2 engineering services on the project and updated a Facility Plan prepared by the City of Lubbock, and retaining the same firm for Step 3 work is in accordance with 40 CFR 35.937-2(b) (2). A contract between the City of Lubbock and Parkhill, Smith & Cooper, Inc., will be negotiated in accordance with applicable portions of 40 CFR Part 35, including appendices; the contract will be negotiated and submitted to the Texas Department of Water Resources. EPA Form 5700-41, Cost or Price Summary Format for Subagreements under U.S. EPA Grants will be submitted along with the engineering contract. Parkhill intends to subcontract construction testing to Terra Testing, Inc., and preparation of an Operation and Maintenance Manual and startup services to Water and Environmental Technology. Terra Testing, Inc., and Water and Environmental Technology are minority business enterprises. Construction Contract will be awarded through competitive bidding in accordance with applicable requirements of 40 CFR Part 35, including appendicies. 4. The project is not located within an identified Flood Hazard Area. 5. No Federal facilities as defined in 40 CFR 35.925-16 will discharge to this wastewater treatment plant. 6. The waste discharge permit for this facility does not require more stringent treatment than secondary. 7. a. Yes, a pretreatment program ,is required under the provisions of 40 CFR 403.8(a). b.
