TOM MARTIN, MAYOR TTEST: Garza, City S PROVED AS TO CONTENT: Brown, Interim Community Development Director PROVED AS TO FORM: EAP res -amend Life Run Independent LivingCenters.I 1 10.10.11 Resolution No, 2011-RO478 STATE OF TEXAS CONTRACTOR OF LUBBOCK AMENDMENT TO COMPREHENSIVE ENERGY ASSISTANCE PROGRAM (CEAP) AGREEMENT BETWEEN THE CITY OF LUBBOCK AND LIFE RUN INDEPENDENT LIVING CENTERS This amendment to Agreement is entered into this 3`d day of November, 2011 between the City of Lubbock, a Texas municipal corporation (hereinafter called "the City") and Life Run Independent Living Centers, a non-profit organization (hereinafter called "Contractor"). WHEREAS, the Contractor and the City have previously entered into an Agreement dated February 10, 2011 (hereinafter called "the Agreement"); and WHEREAS, the City and the Contractor hereby desire to amend said agreement. NOW THEREFORE, the City and the Contractor hereby agree to amend the Agreement as follows: 1) Paragraph II. B. 1. Of the Agreement is hereby amended as follows: City agrees to provide Contractor assistance from Comprehensive Energy Assistance Program (CEAP) funds in an additional amount not to exceed $134,799 in CEAP funds in addition to the $302,500 in LEAP funds previously granted in the agreement and in return for Contractor performing the activities set forth in this Contract as consideration for said funds. 2) This amendment to the Agreement shall be effective upon execution. The Remainder of the Agreement shall remain in full force and affect except as changed herein. Executed the 3rd of November , 2011 as first written as affected as provided herein.
