Resolution No. 2011-RO315 July 27, 2011 Item No. 5.4 RESOLUTION BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF LUBBOCK: THAT Amendments Number Ten (10) and Eleven (11) to the City of Lubbock Employee Benefit Plan are hereby adopted and approved. Said Amendments are attached hereto and incorporated in this resolution as if fully set forth herein and shall be included in the minutes of the City Council. Leisa Hutcheson, Human Res urces Director APPROVED AS TO FORM: Chad Weaver, Assistant City Attorney cdocs/RES.Amend #10 & l l -Employee Benefit Plan 23, 2011 Resolution No. 2011-R0315 City of Lubbock Employee Benefit Plan Plan Amendment No. 10 This Amendment to the City of Lubbock Employee Benefit Plan Document (the "Plan Document") is executed on this, the 27th day of July, 2011, to be effective August 1, 2011. The following change is hereby made to the Plan Document: • Benefits for Coverage of wigs shall be covered at 100%. This benefit is limited to a lifetime maximum of 5500 per participant and will not be subject to calendar year deductible/co-insurance. Additionally, this benefit is payable only for hair loss resulting from chemotherapy, radiation or for loss as a result of medicine induced hair loss; i.e., if a participant is taking medication for hepatitis C and the medicine results in hair loss. This benefit is not intended for naturally occurring baldness in male or female participants. The following change is hereby made to the Plan Document: • Benefits for Tests for Detection of Colorectal Cancer - Benefits for Medical -Surgical Expenses incurred for a diagnostic, medically recognized screening examination for the detection of colorectal cancer, for Participants who are over age 50 years of age or older and who are at normal risk for developing colon cancer, will be determined on the same basis as for other Medical -Surgical Expenses for: o A fecal occult blood test performed annually and a flexible sigmoidoscopy performed every five years; or o A colonoscopy performed every ten years. Each of the following In -Network services shall be paid at 100°x© of Allowable Amount: physician charges; facility charges; and anesthesia charges. This benefit shall be payable with no co -payment, deductible or co-insurance due from the member as long as the results are without a diagnosis of a medical condition.
