Health program. 'The figures set o°.it 'n the program being estimated
costs. The County of Lubbock having agreed to such Health Program
on December 13, 1948.
CITY -COUNTY DEPARTMENT OF PUBLIC 1EALTH
PROPOSED PROGRAM
CITY -COUNTY HEALTH PROGRAM
Since the creation of the office of city physician in 1909, much
progress has been made in the field of Public Health in Lubbock and
Lubbock County. Vre are proud of the progress trade by this depart-
ment and also of the fact that we have one of the leading; departments
in our state. We are, however, assure that our needs of public health
have out grokvn our existina, facilities. It was knowledge of this fact
some six or seven years a.ao that caused us to start an intense study of
health programs and facilities of other leading cities of cur state and
nation. This study has been carried on through personal visitations to
other cities and t'irough questionnaires which were sent to outstanding
derartments that we were unable to reach b y personal coats ct , much in-
formation has been gathered and coml-fled as a result of these studies.
Our aim has been to screen this information, using the most outstanding
workable features, and combining these with proven facilities of our own
department to build what we feel will be a foundation for a model health
program. Facts that were kept constr:ntly before us in the preparation
of the above mentioned program are: first, to study the needs of our
community and build an adequate yet economical program to meet these
needs; second, to so build and organize this program that it may be ex-
panded to meet future needs without radical revisions and needless expen-
ditures; third, to provide ade-ivat, personnel and facilities to carry on
the duties involved in such a health program; and foirth, to provide such
new ordinances as arr-= needed and also make such revisions of existing or-
dinenc{=•s to give proper coverage of present and anticipated ne-ds.
ORGANIZATION
The organization of a City -County Department of tublic Health offers
many advantages, especially from the standpoint of efficiency and econ-
omy. T lis plan avoids the establishment of two administrative units in
a single area and is in conformity with the established fact that the
health of the city affects the entire population of the county and in
turn the health of the city depends upon health of the rural areas
nearby.
The county would be expected to apTropriate only such funds as may be
necessary to care for the increase in expenditures which i%ill result
from expanding a city health program to a city -county program. This
expenditure may be expected o increase as the needs increase but can
be absorbed ,ointly by the city and county or by the agency receivfing
the major benefits. lVe feel thu t by handling the expenses of our
program in this manner each organization will be expending their money
in accordance with the benfits received. There is also a Possibility
that at a later date such organizations as the City County Welfare
Association, the Tuberculosis; Association, Crippled Childrens' Asso-
ciation, and nurses of the dity and cotinty schools could ebmbinE. their
facilities with those of the city -county health program to the mutual
benefit of all concerned.
The sup -port and 000peration of the city -county medical society and the
various interested civic organizations should be earnestiv solicited.
Support of these organizations will be beneficial to both the community
and the department.

The only changes 'n our existing organization as shonnin the preceding
outline will be the addition to our staff of three registered nurses and one
office clerk. This, of course, is based on the assumption that the existing
facilities of the county health department will be utilized.
The organizational chart on the following ra{*e has been designed to show
the functions of this proposed program and set out the existing facilities
and proposed facilities. The existing facilities of the city have been
listed in black with the exception of the two city-sur-orted nurses which
we feel are essential even though tie city continues to operate independent
of the county and, therefore, have been listed as though they wer existing
facilities. The divisions shmvirn in green arc those now supported by the
county and those shoen in red are facilities to b,1 added b.y the county.
The proposed budryet as estimated is shown on the last page and has been so
arranged that the participating organization r,av easily determine his
financial responsibility in the -operation of this program. Present expen-
ditures for both city and county have been listed in black, and the added
expenditures have been listed in red in their respective cblumns. In
studying this proposed budm t, it will be noted that the county share is
approxi natel„v twice their present annual expenditure based on this current
year. owever, their present expenditure does not afford them a full-time
health program but only two employees engage-- in public health work and
participation in the city's DDT s raying program. With the proposed in-
creased expenditures the county will receive the benefits of all existing
facilities of the city health department in addition to nursing service
over the county. The city of Lubbock under this plan will reaeiVe addi-
tional benefits such as nursing services and others that will result from
the operation of a well organized program. The city's expenditure will
be apx roximatle y $15 , 000.00 less than that of the current year. Hov,ever, ,
it should be pointed out that the expenditures for the current year
were necessarily high due to rurchase of an x-ray unit and provision
for rt:organizational salaries preparatory to entering into such a
program as we have proposed. The future ex- :,f the city will
be partial,y covered by proposed charpes for health certificates and
permits of various tyres. In rreparing this budPet, every effort has
been made to anticipate expenditures that will be necessary in a city -
county program; however, some changes and increases ma17 be necessary
as this program progresses. This program has been pr:sented in hopes of
giving Lubbock and Lubbock County an outstanding dull -time health unit
at a reasonable expenditure.
