January 25, 1955 Mayor and City Commission City of Lubbock Lubbock, Texas Gentlemen: I hereby make claim for injuries received on December 28, 1954 as a result of falling in a water -meter hole in front of my residence at 1915 17th Street, Lubbock, Texas. The extent of injury is not definitely known at this time since the knee is stiff and the cut has not healed sufficiently. Encl: Statement of injury attached. Very truly yours, /s/ Lena L. Mattox Lena L. Mattox January 25, 1955 The following is a claim being presented as a result of an accident in front of my residence, 1915 17th Street, on December 28, 1954. On December 28th, 1954 at 5:20 P.M. I got out of my car at my residence at 1915 17th Street, Lubbock, Texas, and upon walking to the house from the street, I stepped in a water meter hole, in my front yard. My leg went down to the bottom of the hole and the lid flew back and hit me on the knee cap, cutting a rather deep gash across the knee cap. I was unable to walk and my husband who was with me at the time took me to the St. Mary of the Plains Hospital at 2605 19th Street. I remained at the hospital until they were able to secure the g Ray technician who had gone home for the day. At 6:15 P.Y. an X Rap was taken of my knee which revealed that the knee had been very badly sprained. Dr. Olan Key whose offices are in the hospital attended me and advised me to go home and stay off my leg, which at the time had begun to swell. I was unable to return to work until January 10, 1955 - a loss of 8 days work at approximately $15.00 per day. The amount of injury at this time cannot be definitely ascertained since the knee is rather stiff at times. X-ray and doctor bill at this time will be approximately 50.00. My address prior to September 13, 1954, was Agana, Guam. My residence from 9-13-54 to 1-15-55 was 1915 17th Street. My resident as of this date of filing this claim is 2601 27th Street, Lubbock, having purchased a home. Nay home telephone # is Sherwood 4-1532. Witness to accident: Paul Y.
